The technical assistance (TA) will support the MOH in the implementation of the Supporting Essential Health Action and Transformation Program with a focus on supporting strategic and innovative actions which will further mainstream climate change mitigation and adaptation measures, gender actions, monitoring and evaluation of nutrition, maternal and child health (NMCH) programs, and digital health reforms. The proposed TA activities have not been included under the planned RBL program boundaries and activities.
The objective of the proposed TA is to support capacity development in the RBL program implementation, particularly activities related to strengthening primary care. The TA will support four outputs to address weaknesses in institutional capacity: (i) gender responsiveness of primary care services increased; (ii) climate responsiveness of primary care services increased; (iii) strategic review of the nutrition, maternal, and child health program effectiveness conducted; and (iv) digital transformation of primary care services accelerated.
Project Rationale and Linkage to Country/Regional Strategy
The Asian Development Bank (ADB) will support the Health System Transformation Agenda (HSTA) with the proposed results-based lending (RBL) Supporting Essential Health Action and Transformation Program loan which will focus on strengthening primary care services by supporting the primary care, health talent, and health technology transformation pillars. This support will help improve the service readiness of primary care and public health laboratories and the widespread adoption of digital tools and inter-operability of health information systems. By supporting the strengthening of primary care, including primary level public health laboratories, ADB acknowledges the critical role of an integrated, well-staffed, and gender- and climate-responsive primary care system in achieving the objectives of the HSTA. A well-functioning primary care system will help prevent severe disease progression, rationalize access and utilization of expensive diagnostics and hospital services, and improve compliance to treatment and medications. It will help address climate change adaptation and mitigation by helping implement relevant health actions of the Enhanced Nationally Determined Contribution of Indonesia. It will also support achieving universal health care, strengthening health security, and improving health outcomes.
The $350 million RBL program will specifically target the pillars on primary care, health talent, and health talent transformation. It will help increase the scope, coverage, quality, and climate- and gender-responsiveness of primary care services. The program will support expanding access to primary care services and improve the quality of services provided by puskesmas, posyandu and pustu, and community health workers (health kaders). It will support the adoption of the integrasi layanan primer or integrated primary care services. It will also support strengthening public health laboratory capacities of primary care facilities, and the adoption of inter-operable digital monitoring, evaluation, and reporting systems for integrated primary health services and public health laboratories. The RBL program will support MOH in strengthening primary care for three years (indicatively 2023 to 2025). |
