The Subject Of The Contract Is The Provision Of Respite Care Services At The Place Of Residence Of A Person With Disabilities Under The Departmental Program Of The Minister Of Family, Labor And Social Policy Term Care - Edition 2025. 2. Services Will Be Provided For A Maximum Of 18 People (16 Adults And 2 Children). The Planned Number Of Hours Of Respite Service Is 4,320 Hours (240 Hours Of Services For 1 Person With Disabilities). 3. Due To The Specificity Of The Subject Of The Contract, The Employer Reserves The Right To Change The Number Of People Using Care Services During The Period Of The Contract. 4. The Contracting Authority Guarantees A Minimum Scope Of The Order: Care For 16 People With Disabilities, I.E. A Minimum Of 3,840 Hours Of Care Services5. A Detailed Description Of The Conditions For The Implementation Of The Subject Of The Contract Is Included In The Planned Provisions Of The Contract Constituting Annex 8 To Swz.6. The Contractor Is Obliged To Have Civil Liability Insurance For The Entire Period For Which The Contract Will Be Concluded In The Scope Of Conducted Activities Related To The Subject Of The Contract For A Guarantee Sum Of Not Less Than Pln 100,000.00 (In Words: One Hundred Thousand Zlotys 00/100), Valid No Later Than From The Date Of Signing The Contract To The Time Of Final Acceptance. The Employer Will Accept The Civil Liability Insurance Document Owned By The Contractor In The Scope Of Conducted Activities Related To The Subject Of The Contract For The Guarantee Sum Required By The Ordering Party, Which Will Be Concluded For The Annual Period And Will Be Renewable While Maintaining The Continuity Of Insurance Protection Until The Final Acceptance. In Such A Situation, The Contractor Is Obliged To Provide The Employer With A Valid Civil Liability Insurance Document In The Scope Of Conducted Activities Related To The Subject Of The Contract Within 7 Days From The Date Of Issuing This Document. 7. The Contracting Authority Sets A Requirement For Employment By The Contractor Or Subcontractor On The Basis Of An Employment Relationship Of Persons Performing The Following Activities In The Scope Of The Contract, If The Performance Of These Activities Consists In Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code. Order Activities Necessary To Perform The Contract, Which Relate To Employment Requirements Based On The Employment Relationship By The Contractor Or Subcontractor Of Persons Performing Activities During The Performance Of The Contract: A Guardian Of A Person With Disabilities.
