Information For Parts/Task No. 1 Service Of Communication Improvement And Attention C-Eyeprzeppra The Object Of The Contract Is The Provision Of Service To Improve Communication And Attention C-Eye In An Amount Of About 210 Consultations For Children Aged 3 Years Old To The Time You Start Studying At Primary School As Part Of A Comprehensive Support Program For Families For Life. The Contractors Duties Will Include: 1) Conducting Diagnosis Of Problems, 2) Conducting Classes For Improving Communication And Attention C-Eye, 3) Keeping Documentation, Including: Specialist Work Cards, Work Cards With The Client, 4) Keeping A Register Of Accepted Persons, 5 ) Performing Actions Subject To The Contract With Due Diligence, 6) Waking Up The Correct Implementation Of This Contract, 7) Providing Services In Accordance With The Schedule, 8) Submitting To The 5Th Day Of The Month A Statement From The Therapy Implemented, Taking Into Account: The Number Of Hours Intended For Service And Numbers People Who Have Used It, 9) In The Event Of A Need - Cooperation With Other Institutions, Entities. As Part Of The Service, The Conducting/Service Provider Is Obliged To Take Part In Meetings, Workshops, Or Additional Team Work Ordered By The Contracting Authority. The Schedule Of Classes Will Be Divided For The Following Stays:- I Stay From The Date Of Signing The Contract- Until April 2025; - Ii Turnus From May 2025 - To August 2025 - Iii Stay From September 2025 - To December 2025, The Setting Description Of The Subject And The Terms Of The Contract Includes A Draft Contract Constituting An Attachment To This Query.
