The Subject Of The Tender Is The Selection Of A Contractor Who Will Provide Supplies To The Ordering Party, Consisting Of Successive Deliveries To The District Hospital. Dr. Tytus Chałubiński In Zakopane Reagents For Biochemical And Immunochemical Tests, Calibrators, Controls As Well As Accessories And Consumables For The Analyzers Referred To In Points 4.8 And 4.9 Of The Swz Together With The Simultaneous Lease Of Two Identical, Fully Automatic Analyzers (The Main Analyzer And A Replacement Analyzer In The Distribution Of Tests 50/50) And Connecting Them To The Laboratory It System Referred To In Point 4.10 Swz. 2. The Ordering Party Stipulates That The Delivered Subject Matter Of The Order Referred To Above Must Have A Current Authorization Required By Law, Authorizing Marketing And Use In Poland, Authorizing Medical Use In Accordance With The Intended Purpose, Including In Particular: 1) Certificate Ce Along With The Declaration Of Conformity, 2) Iso 13485 Certificate (Or Equivalent). The Ordering Party Informs That Whenever The Swz Refers To The Iso Certificate Referred To, For Example, Above In Point 2, The Contractor May Have Equivalent Documents Issued By Entities Based In Another Member State Of The European Economic Area. Moreover, A Contractor Who, For Reasons Beyond His Control, Is Unable To Obtain The Certificate Referred To In Point Above. 2, May Have Another Document Regarding Quality Assurance Or Environmental Management Measures, Respectively, Confirming The Application By The Contractor Of Quality Assurance Measures Consistent With The Required Quality Assurance Standards Or Environmental Management Measures Equivalent To Those Required Under The Applicable Environmental Management System Or Standards. 3. The Ordering Party Stipulates That The Delivered Reagents, Calibrators And Controls To Perform The Number Of Tests Specified In Points 4.8 And 4.9 Of The Swz, Marked With An Expiration Date, Must Have A Minimum Validity Period Of 6 Months, Counted From The Date Of Individual Partial Delivery, Subject To Other Deadlines Specified In Detail In The Tables. Required Parameters. 4. The Ordering Party Stipulates That The Supply Of Reagents For Biochemical And Immunochemical Tests, Calibrators, Controls Along With Accessories And Consumables For The Leased Analyzers For The Duration Of The Contract Will Be Carried Out Successively On The Basis Of Partial Orders Placed By The Laboratory Manager Via Electronic Means Of Communication. 5. The Ordering Party Reserves That The Delivery Of Reagents Covered By The Subject Of The Order Will Take Place Successively, At The Latest Within 7 Calendar Days From The Date Of Placing The Partial Order At From 8:00 To 14:00. 6. Replacement Of Defective Goods, Goods Not In Accordance With The Partial Order, Goods Of Poor Quality, Delivered In Incorrect Packaging Will Take Place Within 48 Hours Of Submitting The Complaint At The Expense And Risk Of
