Speech Medical Services - Neurologopedia Part No. 1 - Covers Approximately 400 Hours, The Place Of Service Provision In Świdnica Or The Świdnica Commune. Years As Part Of The Comprehensive Support Program For Families For Life. As Part Of The Service, The Leader/Service Provider Is Obliged To Take Part In Meetings, Workshops, And Possibly Additional Team Work Commissioned By The Ordering Party. The Planned Schedule Of Classes Will Be Divided Into The Following Periods: - 1St Period From The Date Of Signing The Contract - Until April 2025; - 2Nd Period From May 2025 - Until August 2025 - 3Rd Period From September 2025 - Until December 2025. Contractors Availability: Accepting: • 1-5 Children Per Week. The Contractor Must Be Available At Least Once (One Day) Per Week;• 6-12 Children Per Week The Contractor Must Be Available At Least Twice (Two Days) Per Week;• 13-20 Children Per Week The Contractor Must Be Available A Minimum Of Three Times (Three Days) Per Week;• 21-28 Children Per Week The Contractor Must Be Available A Minimum Of Three Times (Three Days) Per Week;• 29-33 Children Per Week The Contractor Must Be Available A Minimum Of Four Times (Four Days) Per Week. The Contractors Duties Will Include: 1) Diagnosing Problems, 2) Conducting Speech Therapy, 3) Keeping Documentation, Including: Specialist Work Sheets, Work Sheets With Clients, 4) Keeping A Register Of Admitted People, 5) Performing The Activities That Are The Subject Of The Contract With Due Diligence, 6) Supervising The Correct Implementation Of This Order, 7) Providing Services In Accordance With The Schedule, 8) Submitting By The 5Th Day Of The Month A Summary Of The Support And/Or Therapy Provided, Taking Into Account: The Number Of Hours Allocated For Speech Therapy And The Number Of People Who Benefited From It. A Detailed Description Of The Subject Matter And Terms Of Execution Of The Order Is Included In The Draft Contract Constituting An Annex To The Terms Of Reference.
