Request for proposals for Procurement of Construction Works for the Rehabilitation of 16 Preschools in Bosnia and Herzegovina Potential contractors should submit bidding documentation consisting of technical (A) and financial (B) proposals: A) Technical proposal (one separate sealed envelope): 1. Official Company Registration, Tax Registration and VAT Registration documents. Certified copies of original documents defining the constitution or legal status of the company, place of registration, and principal place of business; written power of attorney of the signatory of the Contractor to commit the Contract. 2. List of Works and Projects conducted, outlining 5 years of experience. 3. Scanned, signed and stamped UNICEF Vendor Master Data Form. 4. Bank Statement (BAM) signed and stamped by issuing bank or copy of contract with bank (Proof of company bank account and validity, BAM account). 5. Signed and stamped UNICEF Supplier Profile Form with last Balance Sheets and Income Statements (Bilans stanja i Bilans uspjeha) for the last 3 years. 6. Construction Licences a. FBiH Construction Licence (for LOT 6, 5, 4 and 1), USC Construction Licence for Bosanski Petrovac (under LOT 1) is sufficient. b. RS Construction Licence (for LOT 5, 4, 3 and 2) c. Brko Construction Licence (for LOT 5) 7. Statement on Non-Conflict of Interest. A statement that the company (including all members of a joint venture and Sub-Contractor) is not associated, nor has been associated in the past, directly or indirectly, with the Project for the Project or being proposed as the Project Manager for the Contract. 8. Technical Proposal Submission (Form 1). 9. Technical Proposal Letter (Form 2). 10. Potential Bidder General Information (Form 3). 11. Potential Bidders Contact Details (Form 4). 12. Certified List of Permanent Employees. 13. List of Proposed Key Personnel (Form 5). a. CVs (maximum 2 pages) of listed Key Personnel. b. Diplomas of listed Key Personnel. c. Valid Licences of listed Key Personnel. 14. List of Owned and (or) Leased Machinery and Equipment, necessary to perform the scope of work (Form 6). 15. Potential Bidders Financial Information, Adequacy of Working Capital (Form 7). 16. Evidence of adequacy of working capital for the signed Contract (access to line(s) of credit and availability of other financial resources). 17. Works in Hand and their Financial Values (Form 8). 18. Litigations (Form 9). 19. Proposed Project Implementation plan of Works (Form 10). 20. Minimum 3 references with monetary value of performed works and letters of recommendation.
