1. The Subject Of The Order Is The Successive Delivery Of Disposable Medical Devices - Part No. 1: Epidural Needle - Pediatric, 2. The Subject Of The Order For Individual Parts Is Described In Detail In The Annex To The Swz (Production And Price Form). 3. In The Annex To The Swz, The Ordering Party Requires The Contractor To Provide The Trade Name Of The Product, Catalog Code And Manufacturer Of The Subject Of The Order Offered By The Contractor.4. The Offered Subject Of The Order Must Be Compliant And Approved For Trading And Use In The Territory Of The Republic Of Poland In Accordance With The Requirements Of The Act Of April 7, 2022 On Medical Devices And Regulation (Eu) 2017/745 Of The European Parliament And Of The Council Of April 5, 2017 On Medical Devices, Amending Directive 2001/83/Ec, Regulation (Ec) No 178/2002 And Regulation (Ec) No 1223/2009 And Repealing Council Directives 90/385/Eec And 93/42/Eec. 5. The Subject Of The Order Will Be Delivered At The Expense, Risk And Transport Of The Contractor.
