2. The Subject Of The Order Is The Successive Delivery Of A Drug (Romosozumab) As Part Of The Drug Program To The Provincial Specialist Team In Rzeszów. Dose: 105 Mg/1.17 Ml X 2 Amps. – Str. Quantity In The Package: The Package Contains 2 Pre-Filled Syringes. Quantity: Maximum 600 Packs. The Order Is Carried Out On The Basis Of An Agreement Concluded With The National Health Fund – Agreement No.: 0901001703202405 For The Provision Of Health Care Services – Hospital Treatment – ​​Drug Programs Of February 29 .2024. The Drug Complies With The Announcement On The Reimbursement Drug Program B.160 - Treatment Of Patients With Severe Postmenopausal Osteoporosis. The Ordering Party Reserves The Right To Reduce The Quantity Of The Subject Of The Order Indicated In The Tor, But The Number Of Packages Will Not Be Less Than 300, And In Such A Case The Contractor Will Not Be Entitled To A Claim Against The Ordering Party For Non-Performance Of The Contract, And The Contractor Will Receive Remuneration For The Product Actually Delivered. The Offered Medicinal Products Must Have A Valid Marketing Authorization In The Territory Of The Republic Of Poland Issued By The Minister Of Health Or A Marketing Authorization For Trading Issued By The Council Of The European Union Or The European Commission In Accordance With The Act Of September 6, 2001 Pharmaceutical Law (Journal Of Laws Of 2022, Item 2301). The Ordering Party Allows The Submission Of Equivalent Offers. The Ordering Party Declares That Whenever A Trade Name Was Used In The Tor And In The Annexes, The Ordering Party Did So In Accordance With Art. 99 Section 1 Point 5 Of The Public Procurement Law Only To Precisely Determine The Active Substance Or Composition Of Active Substances Contained In The Ordered Products. The Use Of These Names Means Only That The Ordering Party Requests Products Containing The Same Active Substances Or Compositions Of Active Substances As The Products Whose Names Have Been Used, And Allows The Offer Of Both These Products And Equivalent Products, Which Should Be Understood As Products Identical In Terms Of In Terms Of The Composition Of Active Substances, Purpose And Action. Quality Standards For Medicines Are Specified In Marketing Authorizations.
