The Subject Of The Order Is The Provision Of Catering Services Consisting In The Preparation And Delivery Of: All-Day Nutrition For Patients Of The Provincial Specialist Hospital. J. Koszarowa 5 In Wrocław (Hereinafter Referred To As The Hospital), In A Tray System, Including Children From 5 Months To 18 Years Of Age, Lunches And Afternoon Snacks For Patients Of The Daily Center For Psychiatry And Speech Disorders For Children And Adolescents At Ul. Wołowska 9 (Hereinafter Referred To As The Centre), In The Manner And Under The Conditions Specified In The Description Of The Subject Of The Order, Constituting Appendix No. 1B To The Swz. The Expected Quantities Of Daily Nutritional Portions During The Execution Of The Order Are Specified In Annex No. 1A Assortment And Price Form Swz, I.E.: - 400,000 Person-Days For The Hospital - 12,000 Person-Days For The Center. 1.2. A Detailed Description Of The Implementation Of The Subject Of The Order Is Included In Annex No. 1B And The Draft Agreement, Annex No. 5. 1.3. The Ordering Party Requests That The Contractor Indicate In The Offer The Parts Of The Order The Performance Of Which He Intends To Entrust To Subcontractors And That The Contractor Provide The Names Of The Subcontractors Companies - In Accordance With Art. 462 Section 2 Of The Public Procurement Law, If They Are Already Known. 1.4.
