1. The Subject Of The Order Is The Supply Of Reagents, Calibrators And Other Consumables And Consumables For Hematology, Together With The Lease Of An Analyzer And The Provision Of Service For The Offered Device And Control System For The Laboratory Diagnostics Laboratory Of The Municipal Hospital In Rabka-Zdrój Sp. Z O. O.2. The Order Includes: A) Lease Of The Analyzer For A Period Of 36 Months With Delivery To The Registered Office Of The Ordering Party, I.E. Municipal Hospital In Rabka - Zdrój Sp. Z O. O. Based At Ul. Słoneczna 3, 34 – 700 Rabka – Zdrój, At The Contractors Expense And Risk, Assembly, Commissioning And Commissioning. The Leased Analyzer Must Be Admitted To Trading On The Polish Market, In Accordance With Applicable Law. Detailed Technical Parameters, Including Description And Requirements, Are Included In Annex No. 1 To The Swz. This Description Should Be Read Together With Any Changes To The Content Of The Swz, E.G. As A Result Of Responses To The Contractors Inquiries. B) Staff Training In The Operation And Maintenance Of The Leased Device And The Interpretation Of Test Results. The Ordering Party Requires Training To Be Carried Out Within 2 Business Days From The Date Of Installation Of The Device. C) Successive Delivery Of Reagents And Necessary Consumables (As Needed) To Perform Tests In The Quantities Described In Annex 2 To The Swz. The Contractor Is Obliged To Offer All Necessary Reagents For The Specified Tests In The Tender Offer. The Given Quantities Are The Estimated Demand During The Duration Of The Order Used To Calculate The Offer Price, I.E. Determine The Maximum Remuneration Of The Contractor. The Purchase Of Each Product Will Depend On The Actual Needs Of The Ordering Party. The Ordering Party Reserves The Right To Change The Scope Of The Subject Of The Order By Reducing It, And The Ordering Party Guarantees The Implementation Of The Subject Of The Order At A Level Not Lower Than 50% Of The Size Of The Subject Of The Order. The Ordering Party Reserves The Right To Change The Quantity Of The Ordered Product Between Individual Items Of The Subject Of The Order. Delivery Of The Subject Of The Order Will Take Place Successively, In Quantity And Assortment Consistent With The Ordering Partys Needs, At The Expense And Risk Of The Contractor, Using Its Own Transport Or Through A Professional Carrier Or Forwarder To The Ordering Partys Headquarters.
