The Subject Of The Order Is The Purchase And Delivery Of An Mri Scanner With Equipment And The Adaptation Of Rooms For The Needs Of The District Health Center - Independent Public Team Of Specialized Clinics, Ul. Szpitalna 6A, 87 - 800 Włocławek. 2. A Detailed Description Of The Subject Of The Contract Is Included In Annex No. 1 To The Swz - Description Of The Subject Of The Contract. The Contractor Will Attach The Completed And Signed Annex No. 1A To The Swz - List Of Technical And Operational Parameters To The Offer For The Purpose Of Evaluating The Offers In The Additional Offer Evaluation Criterion - Technical Parameters And To Confirm That The Offered Supplies Meet The Minimum Technical And Operational Parameters. Detailed Requirements For The Implementation Of The Subject Of The Order Are Set Out In Annex No. 2 To The Swz - Draft Provisions Of The Contract. 3. Warranty Period - Minimum 24 Months. The Offered Subject Of The Order Must Be Approved For Marketing And Use In Health Care Units In The Territory Of The Republic Of Poland In Accordance With Polish And Eu Law. Must Have Valid Polish Or Eu Certificates/Certificates Of Approval For Use In Medical Entities - If Applicable. 7. The Subject Of The Order (Magnetic Resonance Imaging) Will Be A Medical Device Within The Meaning Of The Act Of April 7, 2022 On Medical Devices And Will Meet All The Requirements Specified In This Act And The Regulations Issued On Its Basis. 8. The Contractor Will Perform The Subject Of The Order In Accordance With Applicable Law, In Particular The Act Of April 15, 2011 On Medical Activities And The Act Of November 29, 2000, Atomic Law, Including Installation, Servicing, Calibration, Inspections, Which Will Be Performed By Authorized Persons In Accordance With The Act Of April 7, 2022 On Medical Devices. 9. The Subject Of The Contract Will Be Performed In Accordance With Polish Standards And Legal Provisions. Adaptation Works Must Be Performed In Accordance With The Requirements And Applicable Regulations, In Particular The Requirements Of The Act Of July 7, 1994 Construction Law, Health And Safety Regulations When Performing This Type Of Work And In Accordance With The Principles Of Technical Knowledge, Documentation And Due Diligence In Their Execution, Good Quality And Proper Work Organization. 10. The Contractor Will Have Appropriate Knowledge, Experience, Qualifications, Specialization, Tools And Software Necessary To Properly Perform The Subject Of The Order And There Are No Legal Or Factual Obstacles To
