The Specialized Care Service Provided By A Physiotherapist Includes Physical Rehabilitation, In Accordance With The Specifications - Requirements Described Below, For Residents Of The Połczyn-Zdrój Commune, Participants Of The Project Recipe For Support - Development Of Social Services In The Commune Area. Połczyn-Zdrój As Part Of The European Funds For Western Pomerania 2021-2027 Program. As Part Of The Service, The Contractor Is Obliged To: Provide A Specialized Care Service Including Physical Rehabilitation In A Total Estimated Amount Of 1,624 Hours For People Living In The Połczyn-Zdrój Commune - Participants Of The Project, In Period From The Date Of Signing The Contract To December 31, 2026, According To The Following Principles: - Implementation Is Assumed Specialized Care Service For 7 People Requiring Support (Oww) For 8 Hours Per Month, Which Allows Us To Estimate The Average Involvement Of A Physiotherapist At 56 Hours Per Month. The Specialized Care Service Including Physical Rehabilitation Is To Be Provided At The Project Participants Place Of Residence (Mobile Service) In The Połczyn-Zdrój Commune. Specialized Care Services Should Be Provided By A Physiotherapist Who Will Adapt Their Scope To The Diagnosed Needs Of Individual Project Participants, Based On A Medical Certificate And An Interview With The Project Participant Or His/Her Guardian. The Physiotherapist Will Carry Out Physical Rehabilitation And Improve Disturbed Body Functions In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, Which Will Include In Particular: - Individual Exercises, - Manual Massage, In The Scope Of Activities Resulting From The Needs Related To The Consequences Of Diseases, - Physiotherapy. To Properly Perform The Order, The Contractor Should Have Its Own Equipment, In Particular Devices For Physiotherapy Treatments And A Means Of Transport To Provide The Service In A Mobile Form. The Ordering Party Will Provide A Set Consisting Of Resistance Bands, Rehabilitation Balls, Rollers And Sensory Accessories. While Performing The Service, The Contractor Will Be Obliged To Protect Personal Data In Accordance With The Act Of May 10, 2018 On The Protection Of Personal Data And The Provisions Of The Gdpr. Specialized Care Services Must Be Provided Based On An Established Individual Assistance Plan For A Given Client Prepared By The Contractor. The Contractor Will Settle The Services Provided On The Basis Of A Monthly Card Of Visits To The Community, Taking Into Account The Scope Of Services Provided - Annex No. 5. Monitoring Of The Services Will Also Be Performed Using Electronic Tools.
