Tenders are invited for Supply of Printed Stationery (Printed File Covers). The relevant application forms could be obtained from the Accounts Division of this hospital on working days of the week from 03.12.2024 until 30.12.2024 between 9.00 a.m. and 3.00 p.m. by submitting the receipt obtained after paying the non-refundable fee of Rs. 1,000/- to the Shroff of Ampara General Hospital as Registration Fee for each category (for each supply/service). (Money orders/ stamps or cheques will not be accepted). Applications should be enclosed in an envelope and sealed. It is mandatory that the photocopy of the Registration Certificate is submitted. Applications will be accepted up to 10.30 a.m. on 31.12.2024 and they may either be sent by registered post addressed to "Director, General Hospital, Ampara or deposited in the Tender Box kept in the Office of the Accountant. The caption, Registration of Suppliers and Building Maintenance Contractors and Building Contractors for year 2025 should be mentioned on the left-hand top corner of the envelope containing the application. Applications will be opened at 10.30 a.m. on the same day in the presence of Procurement Committee of this hospital.
