Health And Social Work Services – Provision And Coordination Of Specialized Services For Participants Of The Project Wielkopolskie Telecentrum Dobre_2. The Subject Of The Order Is: Provision And Coordination Of Specialized Services For Participants Of The Wielkopolska Care Telecenter Project Co-Financed By The European Union Under The European Funds For Greater Poland 2021-2027 Program - Priority 6, Action 06.13 Social And Health Services. The Subject Of The Order Includes: 1. Provision And Coordination Of Specialized Services In The Form Of Teleconsultations, Which Will Be Implemented Using The Social Coordination Platform Made Available To The Contractor By The Ordering Party, Hereinafter Referred To As Pkos (The Description Of The Substantive Assumptions Of The Pkos Platform Is Attached As Annex No. 1 To Opz), In The Scope Of: A ) Psychologist B) Dietitian C) Gerontologist Or Psychogerontologist D) Psychosexologist 2. Form Of Providing Specialized Services: 2.1. The Provision Of Specialist Services In The Form Of Teleconsultations Will Be Carried Out Using A Telephone Switchboard, After Prior Arrangement Of The Technical Data Of The Switchboard With The Ordering Party (Necessity Of Integration With Pkos), Confirming The Time Of Specialists Work, With The Possibility Of Printing Monthly Reports (Including Data On The Specialist, Date , Hour, Call Duration, Comments). The Telephone Exchange Will Have One Telephone Number Common To All Specialists, Which Will Be Visible When Making Calls. 2.2. Specialists Will Provide Specialist Services As Part Of Teleconsultations, Which Will Be Billed For The Actual Time Of The Teleconsultation, Based On The Rate Indicated By The Contractor For A Given Specialist. In The Case Of Seconds, The Time Will Be Rounded Up To The Next Full Minute. Remuneration For The Specialists Work Will Be Settled Per Minute, Based On The Time Of The Teleconsultation. Example: Teleconsultation Lasts 24 Minutes And 34 Seconds, The Settlement Should Include The Teleconsultation Time: 25 Minutes, And The Remuneration Will Be: 25 Minutes Times The Price Per Minute Of Teleconsultation Provided By The Specialist. 2.3. Specialists Will Have Access To The Contact Details Of Project Participants Who Use Teleconsultation, The Ability To View Data On Completed Teleconsultations, And Will Contact The User Of The Wristband Scheduled For Teleconsultation By Phone. 2.4. The Contractor Will Be Obliged To Provide A Monthly Calendar (Available Online) In Which The Monthly Schedule Of Availability Of Specialists Providing Teleconsultations Will Be Presented. The Contractors Obligation Will Be To Submit A Calendar With A Schedule For The Coming Month No Later Than The 5Th Day Of The Current Month. This Calendar Will Include The Next Full Month Of Services Provided By Specialists And Will Precisely Indicate The Dates And Times When Each Specialist Will Be Available To Conduct Teleconsultations. The Calendar Provided By The Contractor Must Meet High Standards Of Protection Of Personal Data Of Project Participants, Ensuring Full Confidentiality Of This Information. 2.5. The Coordinator Will Be Obliged To: • Enter Available Dates In The Pkos System
