The Subject Of The Order Includes The Contractors Provision Of A Physical Inventory Service Of Medical Equipment And Other Fixed Assets, Including Equipment Marked With Rfid Technology, Together With The Medical Equipment Management System (Hereinafter Referred To As The System) And Inventory Accessories. Design Assumptions: - Total Quantity Of Medical Equipment For Inventory: Medical Equipment Approx. 2,300 Pcs. - Other Fixed Assets And Inventory Equipment, Divided Into 2 Groups: · Electronic (Monitors, Computers, Air Conditioners, Etc.) Electronic, Technical And Specialized Machines And Devices Approx. 1,200 Pcs. · Non-Electronic (Cabinets, Chairs, Desks, Etc.) Non-Electronic Approx. 500 Pcs. Buildings And Infrastructure Approx. 90 Pcs. Property Location: Ul. Koszarowa 5, Wrocław - 4 Hospital Buildings, Including 3 Adjacent To Each Other; 13 Administrative And Technical Buildings Ul. Wołowska 9, Wrocław- 1 Medical Building; 1 Technical Building2. The Scope And Description Of The Subject Of The Order Are Set Out In Annex No. 2 To The Swz - Opz And The Conditions Of Its Implementation, The Ordering Party Specified In The Draft Contract, Which Constitutes Annex No. 4 To This Swz.3. Employment Requirements Based On The Employment Relationship: 1) Pursuant To Art. 95 Of The Public Procurement Act, The Ordering Party Requires The Contractor Or Subcontractor Or Further Subcontractor To Employ, Under An Employment Relationship, Persons Performing Work That Includes The Following Types Of Activities Covered By The Subject Of The Contract: I.E. Inventory Activities, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code (Consolidated Text: Journal Of Laws Of 2022, Item 1510), Unless They Will Be Directly Performed By A Given Person As Part Of His Or Her Sole Proprietorship.
