The Subject Of This Order Is To Provide Services Consisting In The Preparation And Delivery Of Daily Meals Prepared In The Contractors Kitchen To Zama For Patients Using 24-Hour Health Care Services At Wszpzoz Ochrony Zdrowotnej Im. Prof. E. Wilczkowskiego In Gostynin, Hereinafter Referred To As: Wszpzoz Gostynin-Zalesie, Mental Health Center - Wards: Iii C, V C, Vi C And The Psychiatric Day Ward, With The Reservation That The Psychiatric Day Ward Will Only Use Lunches. 2. As Part Of This Contract, Wyk Undertakes To Provide Zam With A Service Consisting In The Preparation And Delivery To Zam Of All-Day Meals Prepared On The Basis Of The Wyk Kitchen For Patients Using 24-Hour Health Services At Wszpzoz In Gostynin-Zalesie, Mental Health Center - Branches: Iii C, V C, Vi C And The Psychiatric Day Ward, Provided That The Psychiatric Day Ward Will Only Use Lunches, Assistance In Unloading Onto Ward Trolleys, Washing And Disinfection Of Gn Containers And Thermoports In The Wyk Rooms On Its Own, And Collection And Disposal Of Food Waste , With Due Diligence, In Compliance With Legal Provisions, Occupational Health And Safety And Sanitary-Epidemiological Regulations, In Accordance With The Contract Constituting Annex No. 2 To This Swz, Which Is Its Integral Part, Form Of Offers Wyk, Constituting Annex No. 1 To This Swz / Annex No. 4 To The Contract , Which Is An Integral Part Thereof, And This Swz Together With All Its Contents, Excluding Appendices 5-7. 3. Before The Order Referred To In Section 1 Of This Includes The Obligations Of The Contract Including: 1) Preparation And Deliveries To Zams Branches Referred To In Section 5 Of This Meal Plan Taking Into Account A Basic, Easily Digestible, Low-Fat, Low-Cholesterol, Dairy-Free, Low-Protein Diet, Including 3 Meals A Day: Breakfast, Lunch And Dinner, And For The Following Diets: Diabetic, With Limited Substances Stimulating The Secretion Of Gastric Juice, Reducing, High-Protein, Low-Fat - Pancreatic , High-Calorie, Including 5 Meals A Day, I.E. Breakfast, Lunch, Dinner, Second Breakfast, Afternoon Snack And An Additional Night Meal In A Diabetic Diet, 7 Days A Week Twice A Day (I.E. Breakfast And Second Breakfast 7:30-8: 15 And Lunch, Afternoon Tea And Dinner 12:45 - 13:30), In A Thermos System, I.E. Using Specialized, Certified Thermoports, Gn Containers Or Equivalent, Ensuring The Maintenance Of Optimal Temperature And Quality Of Meals During Transport From The Wyk Kitchen; 2) Preparation And Delivery Of Meals In The Form Of Lunch To The Psychiatric Day Ward, Taking Into Account The Diets Referred To In Section 3 Point 1) Of This Agreement, For 5 Working Days A Week, Excluding Holidays And Public Holidays, Once A Day (I.E. .: Lunch 12:45 - 13:30), In A Thermos System, I.E. Using Specialized, Certified Thermoports, Gn Containers Or Equivalent, Ensuring The Maintenance Of Optimal Temperature And Quality Of Meals During Transport From The Kitchen.
