The Subject Of The Order Is: Provision Of Medical Services Including Preventive Medical Examinations (Initial, Periodic, Control) And Sanitary And Epidemiological Examinations, Diagnostic Tests, Laboratory Tests, Psychotechnical Tests, Psychological Tests, Specialist Consultations For Professional Soldiers And Military Employees Employed In 45 Wog Wędrzyn And The Units And Institutions Supplied By It, I.E.: 17Th Mechanized Brigade Międzyrzecz, 35Th Air Defense Missile Squadron Skwierzyna, Land Forces Training Center Wędrzyn, Military Police Department Wędrzyn, Military Recruitment Center Gorzów Wielkopolski, 6Th Radio Targeting Platoon Skwierzyna, Wędrzyn Depot, Skwierzyna School Rocket Repair Workshop (Nowogród Bobrzański Depot), Wrocław District It Center (Wędrzyn, Międzyrzecz, Skwierzyna), Delegation Of The Military Transport Command Wrocław, 151St Light Infantry Battalion Skwierzyna (12 Wot). The Expected Estimated Number Of Soldiers And Military Employees To Perform Occupational Medicine Examinations In 2024 Is Over 3,720 People. This Number May Increase Or Decrease Due To Changes In Employment Levels And Medical Check-Ups Of Military Employees And Soldiers. The Contractor Implementing The Subject Of The Order Must Have Medical Apparatus And Equipment As Well As Premises Guaranteeing A Full Range Of Medical Services That Meet The Requirements Set Out In The Act Of April 15, 2011 On Medical Activities Regarding Premises And Equipment Of An Entity Performing Medical Activities (Journal Of Laws 2023.0.991, Consolidated Text). Ordering Party Stipulates That The Maximum Distance Of The Tests Performed May Not Exceed 50 Km From The Ordering Partys Headquarters (Wędrzyn). The Subject Of The Order Will Be Implemented In Accordance With The Regulation Of The Minister Of Health And Social Welfare Of May 30, 1996 On Medical Examinations Of Employees, The Scope Of Preventive Health Care For Employees And Medical Certificates Issued For The Purposes Provided For In The Labor Code (Journal Of Laws Of 2023, Item 607, Consolidated Text), (Diagnostic Tests, Laboratory Tests, Psychotechnical Tests, Psychological Tests, Specialist Consultations), In Particular: 1) Performing Preventive Medical Examinations Of Professional Soldiers And Military Employees - Initial, Periodic And Control Examinations, Hereinafter Referred To As Preventive Examinations;2) Issuing Medical Certificates For The Purposes Provided For In The Labor Code And Implementing Regulations;3) Performing Examinations Of Drivers;4) Providing Other Health Services Which Are To Be Provided Pursuant To The Employer Is Obliged To Comply With Separate Regulations; 5) Perform Laboratory Tests And Other Tests Specified In The Table (§ 1 Section 5 Of The Contract Template) Ordered By An Occupational Medicine Doctor; 6) Taking X-Rays Ordered By An Occupational Medicine Doctor; 7) Preparing Test Descriptions; 8) Conducting Consultative, Diagnostic And Therapeutic Activities In The Field Of Occupational Pathology; 9) Conducting Active Counseling For Patients For Diseases Related To Work/Service Performed;10) Monitoring The Health Condition Of Workers In Special Risk Groups;11) Collecting And Storing (In Accordance With The Regulations) Information On Occupational Risk And The Health Condition Of People Under Preventive Medical Care;12) Ensuring The Participation Of A Doctor, Nurse Providing Preventive Health Care To Employees In The Work Of The Occupational Health And Safety Committee And In The Review Of Job Positions After Written Notification By The Ordering Party 5 Days Before The Planned Date Of The Committee Meeting Or Job Review: 13) Carrying Out Medical Examinations For Sanitary And Epidemiological Purposes By Doctors In Accordance With The Regulations Act Of December 5, 2008 On Preventing And Combating Infections And Infectious Diseases In Humans (Journal Of Laws 2023.1284 Vol.);14) Carrying Out Medical And Psychological Examinations Of Persons Applying For A License And Holding A Physical Security Employee License In Accordance With The Act Of August 22, 1997 On The Protection Of Persons And Property (Journal Of Laws 2021.1995 Vol. J);15 ) Performing Psychological Tests Of Persons Applying For Driving Licenses, Drivers And Persons Performing Work As Drivers In Accordance With The Regulation Of The Minister Of Health Of July 8, 2014 (Journal Of Laws Of 2022, Item 165, Consolidated Text). 16) Regulation Of The Minister Of Health Of April 15, 2011 Amending The Regulations On Medical Examinations Of Drivers And Persons Applying For Driving Licenses (Journal Of Laws 2011.88.503). The Ordering Party Does Not Bear The Financial Consequences Of Reducing The Number Of Examinations. The Contractor Is Entitled To Remuneration For The Research Actually Carried Out. The Contract Templates (Annex No. 6 To The Swz) Supplement The Description Of The Subject Of The Contract.
