Miscellaneous Repair And Maintenance Services - Maintenance And Minor Repair Work In Residential Municipal Buildings.. 1. The Subject Of The Order Includes: 1) Ongoing Inspection Of The Condition Of Building Equipment, Such As Joinery, Cladding Of Staircases, Landings And Other Internal Ones Pedestrian Routes, 2) Ongoing Inspection Of The Technical Condition Of Pedestrian Routes. 3) Checking Condition Of Walls And Plasters Of Staircases, Basements And Facades Once A Quarter Or Whenever Necessary Receiving A Notification From The Ordering Party; 4) Checking The Status And Operation Of The Installation Intercoms In The Event Of Receiving A Notification From The Ordering Party/Tenant; 5) Control Operation Of Door Closers, Building Fittings, Locks And Door Handles, At Least Once A Day Month; 6) Maintenance, Repair And Checking The Efficiency Of Windows In Buildings, Z Except For Windows In Residential Premises, Including Improving Tightness And Efficiency Window Operation, At Least Once A Month And At The Request Of The Ordering Party, Regulations Fittings And Troubleshooting; 7) Immediate Replacement Of Broken Glass In Windows And Doors Of Common Areas Of Buildings; 8) Maintenance And Repair Of Storage Boxes Post Offices And Replacing Their Locks, Without Replacing The Boxes Themselves; 9) Maintenance And Repair Of Doors To All Common Areas In Buildings, Including Lubrication Hinges, At Least Once A Month, Removing Jams And Malfunctions On An Ongoing Basis Locks And Handles; 10) Maintenance And Repair Of Garbage Sheds, Fences, Repair/Replacement Septic Tank Flaps, Repairing Defects In Entrance Gates And Roofing And Sanitary Coverings; 11) Immediate Notification Of City Services In Case Of A Threat Safety And Life Of Residents; 12) Maintenance And Repairs Other Than Those Mentioned In This Description Of The Subject Of The Order, Mechanical Equipment In Buildings And On Adjacent Areas, Including Cleaning Rainwater Containers; 2. Detailed The Description Of The Subject Of The Order Is Included In Annex No. 1 To The Swz And In The Designed Documents Provisions Of The Contract. 3. The Ordering Party, In Accordance With The Requirements Of Art. 95 Section 1 Of The Act Specifies The Following Requirements Regarding The Employment Of People By The Contractor Or Subcontractor Based On An Employment Contract: 1) Activities Indicated In Points 1-16 Of Part I Of The Subject Description Orders. 2) Method Of Verifying The Employment Of Persons Referred To In Art. 95 Section 1 Of The Act Was Specified In The Draft Provisions Of The Agreement, Constituting An Annex No. 6 To Swz. 3) The Ordering Partys Rights To Control The Contractors Compliance Requirements Referred To In Art. 95 Section 1 Of The Act And Sanctions For Failure To Comply These Requirements Have Been Specified In The Draft Provisions Of The Contract Annex No. 6 To The Swz. 4. The Contractor Is Obliged To Submit Declarations And Documents On The Dates Indicated Below: Submitted With The Offer In The System - On The Purchase Platform 1) Completed And Signed Offer Form - According To The Template Attached As Annex No. 2 To Swz. 2) Declaration Of The Contractor/Entity On The Resources On Which The Contractor Relies / Contractors Jointly Applying For The Award Of A Contract Not To Be Excluded From The Proceedings, Meeting The Conditions For Participation In The Proceedings - On The Unified Form European Procurement Document (Edz) - The Declaration Template Is Attached As Attachment No 3 To Swz; 3) Declaration Of The Contractor/Entity On The Resources On Which The Contractor/Contractors Rely Jointly Applying For The Award Of The Contract, Confirming That The Contractor Shall Not Be Excluded From The Proceedings On The Basis Of The Conditions Specified In Art. 7 Section 1 Of The Act Of April 13, 2022 On Special Solutions In The Field Of Counteracting Support For Aggression Against Ukraine And Protecting Security National, According To The Template Specified In Annex 7 To The Swz; In Case The Contractor Relies On The Abilities Or Situation Of Other Entities, Attaching To The Offer In The System: Statements Regarding Each Entity; 4) Declaration Of The Contractor/Entity On The Resources, Which The Contractor/Contractors Jointly Applying For The Contract Rely On Constituting Confirmation That The Contractor Is Not Subject To The Prohibition Specified In Art. 5K Council Regulation (Eu) No 833/2014 Concerning Restrictive Measures In Connection With Russias Actions Destabilizing The Situation In Ukraine, According To The Pattern Set Out In Annex No. 8 To The Swz; Where The Contractor Relies On Ability Or Situation Other Entities, Attaching To The Offer In The System: Statements Regarding Each Entity. 5) A Deposit Document, If Paid In A Form Other Than Money (V Case Where Required); 6) Document Or Documents From Which It Will Result Scope Of Authorization If The Offer Is Signed By Person(S) On Behalf Of The Contractor Other(S) Than Those Disclosed In The Registration Documents; 7) Power Of Attorney For The Representative To Represent Contractors Jointly Applying For The Award In The Proceedings Procurement – Applies To Offers Submitted By Contractors Jointly Applying For Awarding The Contract; 8) Obligation Of The Entity Providing Resources To Donate The Necessary Resources At The Contractors Disposal For The Purpose Of Executing The Order - Requirement Submission In The Event That The Contractor Relies On The Abilities Or Situation Of Entities Providing Resources - The Commitment Template Is Attached As Annex 4 To The Swz; 9) Statement Submitted Pursuant To Art. 117 Section 4 Of The Act, Which States Which Services They Will Perform Individual Contractors - Submission Requirement If Contractors Apply Jointly For The Award Of A Contract - The Declaration Template Is Attached As Annex 5 To The Swz. 5. Other Conditions Of Participation And Grounds For Exclusion Are Specified In The Specification Terms Of The Order And In The Draft Provisions Of The Agreement.
