Tenders are invited for Supply of Hospital Beds, Lockers, Trolleys, TV Stands, Wheel Chairs and Similar Equipment for Wards. Applicants intending to register for more than one item should mention each item and the application fee for each item is Rs. 1,000.00 and the application fee should be credited to Account No. 2026181 maintained at the Bank of Ceylon Kalutara Branch in favour of Regional Director of Health Services and the original receipt obtained should be submitted with application prepared as per the specimen along with the certified copies of the Business Registration Certificate and the VAT Registration Certificate. All applications may either be sent by registered post addressed to "Office of the Regional Director of Health Services, District Secretariat Building, P.O. Box 29, Kalutara" or hand delivered to receive on or before 06th February 2024. The caption, "Registration of Suppliers for Year 2024" should be mentioned on the left-hand top corner of the envelope containing the application.
