The Subject Of The Order Is The Organization And Provision Of Individual Personal Assistance Services For People With Disabilities Qualified By The Ordering Party As Part Of The Project Support For The Development Of Social Services In The Giżycko Commune. The Basic Order Covers A Maximum Of 2,172 Hours Of Services In The Period From January 1, 2027 To December 31, 2027. The Ordering Party Guarantees The Order Of At Least 1,629 Hours, Constituting 75% Of The Basic Scope. The Option Right Covers A Maximum Of 3,078 Additional Hours In The Period From January 1, 2028 To May 31, 2029. The Total Maximum Scope Of The Order Is 5,250 Hours. It Is Estimated That 10 People Will Benefit From The Support Per Month, And A Total Of 30 People Throughout The Entire Order Implementation Period. The Same Person Can Benefit From Support In Subsequent Months. The Number Of Participants Is Estimated. The Scope, Frequency And Number Of Hours Of Support Result From The Individual Needs Of Participants. Services Will Be Provided Primarily At The Participants Place Of Residence In The Giżycko Commune, As Well As In Other Places Resulting From Their Individual Needs, On The Terms Specified In The Opz. The Contractor Ensures The Organization And Continuity Of Services, Appropriate Staff, Replacements, Coordination, Quality Supervision And Documentation Of Support. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 2 To The Swz, And The Rules For The Implementation And Settlement Of Services Are Specified In The Draft Provisions Of The Contract Constituting Annex No. 8 To The Swz.
