Tenders are invited for Supply of Hospital Equipment: Hospital Beds, Trolleys, Food Trolleys, Saline Stands, Oxygen Stands 04.01. An non-refundable registration fee of Rs. 2,000.00 per item must be paid (Note: The text states Rs. 2,000 per item, though the example calculates as: 10 items × Rs. 500.00 = Rs. 5,000.00). Payments can be made in cash to the Shroff at Teaching Hospital - Polonnaruwa until 3.00 PM on 02.12.2026, or credited to Account No. 7040277 in the name of Director, Teaching Hospital, Polonnaruwa at the Bank of Ceylon, Kaduruwela Branch. The original cash payment receipt must be attached to the application form. 04.02. When submitting applications, it is mandatory to include supporting documents verifying legal status, storage capacity, financial capability, and agency/dealership details for supplying goods under each category. 04.03. Separate applications must be submitted for each item. (Applications for Medical Items must be obtained directly from the hospital). All applications should be duly completed according to the given format and sent via registered post to reach the Chairman, Regional Procurement Board, Teaching Hospital, Polonnaruwa on or before 2:00 PM on 03.12.2026, or deposited in the procurement box kept at the Accounts Division. The top left corner of the envelope must be marked Registration of Suppliers - 2027. Late applications will not be accepted. Note: The notice contains a fee inconsistency in section 04.01: it states Rs. 2,000 per item but gives an example of 10 items × Rs. 500 = Rs. 5,000.
