Kołłątaja 31.. 1. The Subject Of The Order Is The Preparation Of Design And Cost Estimate Documentation For The Renovation And Reconstruction Of A Municipal Building In The Scope Of Construction Of An External Shaft Of A Passenger Lift (Elevator) At Ul. Kołłątaja 31 In Wrocław. The Subject Of The Contract Is Divided Into 2 Stages, Where: 1) Stage 1 - Is A Guaranteed Stage, 2) Stage 2 - Is A Stage Covered By The Option Right, In Accordance With Art. 441 Of The Public Procurement Law. 2. The Scope Of The Order In The 1St Stage (Guaranteed) Includes, Among Others: 1) Building Inventory, Stratigraphic Examination Of The Facade, Ornithological And Chiropterological Opinion, 2) 2 (Two) Different Concepts For The Reconstruction Of The Building In Terms Of Adapting It To The Needs Of People With Disabilities (Construction Of An Elevator), 3) Agreeing With The Lower Silesian Provincial Conservator Of Monuments On The Concept Selected By The Ordering Party, 4) Design Architectural And Construction Project Together With A Plot Or Area Development Plan/Site Plan Showing The Location Of The Investment (Hereinafter Referred To As The Architectural And Construction Project). 3. The Scope Of The Order In Stage Ii (Option Right) Includes, Among Others: 1) Preparation Of Design And Cost Estimate Documentation, Including: A) Multi-Discipline Technical Designs, B) Bills Of Quantities, C) Investor Cost Estimates, D) Technical Specifications For The Execution And Acceptance Of Construction Works (Hereinafter Referred To As Stwiorb), 2) Obtaining And Submitting To The Ordering Party The Decision Of The Department Of Architecture And Monuments Of The Wrocław City Hall (Hereinafter Referred To As Waiz Um) Building Permit Along With Confirmation Of Submitting An Application To Waiz Um For Issuing A Certificate That The Decision Has Become Final/Obtaining And Providing The Ordering Party With A Certificate Confirming The Lack Of Grounds To Object To The Intention To Perform Construction Works In Accordance With Art. 30 Section 5Aa Of The Act Of July 7, 1994 Construction Law, 3) Performing Authors Supervision During The Implementation Of Construction Works Covered By The Developed Documentation. 4. The Deadline For The Execution Of The Order Is: 1) Stage I (Guaranteed): Up To 5 Months From The Date Of Conclusion Of The Contract, 2) Stage Ii (Covered By Option Law): A) Up To 11 Months From The Date Of Conclusion Of The Contract - Preparation And Submission Of Design And Cost Estimate Documentation And Obtaining And Forwarding To The Ordering Party The Decision Of Waiz Um Of A Building Permit Along With Confirmation Of Submitting An Application For A Certificate That The Decision Has Become Final/Certificate Of No Grounds To Object To The Intention To Execute Construction Works, B) Authors Supervision Over The Implementation Of Construction Works Covered By The Developed Design And Cost Estimate Documentation - For The Duration Of The Construction Works, But No Longer Than For A Period Of 36 Months From The Date Of Obtaining The Final Decision On The Building Permit/Certificate Of No Grounds To Object To The Intention To Perform Construction Works.
