Prior Information Notice For X-Ray Devices – Skiagraphic X-Ray Devices Including Full Service Provision. The Subject Of The Public Contract Is The Gradual Supply Of 3 X-Ray X-Ray Devices Including Accessories And Related Technologies (Hereinafter Also Referred To As Device, Goods, Medical Device/S Or Simply Zp), The Provision Of Warranty Activities For The Clients Radiology Clinic, Specifically For The Pavilion Of Internal Fields (Hereinafter Also Referred To As Pio) And The Academician Bedrna Pavilion (Hereinafter Referred To As Mcho), On The Basis Of A Purchase Contract, Under The Conditions Set In The Tender Documentation. The Subject Of This Public Contract Also Includes: • Provision Of Post-Warranty Full Service Of Individual Goods For An Indefinite Period, Based On A Service Contract, Under The Conditions Specified In The Procurement Documentation. • Gradual Disassembly And Take-Back Of Refurbished Equipment (3 X-Ray Skiagraphic Sets Including Accessories) By The Client, On The Basis Of A Work Contract For Disassembly And Take-Back, Under The Conditions Specified In The Procurement Documentation. O 1 Pc At The Central X-Ray (Building No. 2) – X-Ray Machine Gmm Incl. Accessories From 2003. O 1 Pc Per Pio (Building No. 10) – In The Skelet Examination Room It Is An X-Ray Set Toplift (Hv Generator Cpi Indico 100, Varian X-Ray Machine, ...) Incl. Accessories From 2016. O 1 Pc Per Pio (Building No. 10) – In The Lung Examination Room It Is An X-Ray Set Toplift (Hv Generator Cpi Indico 100, Varian X-Ray Machine, ...) Incl. Accessories From 2016. • Provision Of Defined Building Modifications For Individual Pieces Of Goods (2 Pieces For Pio), On The Basis Of A Work Contract For Building Modifications, Under The Conditions Specified In The Procurement Documentation. The Offered Goods Must Be Placed On The Market In Accordance With Applicable Legal Regulations, I.E. Regulation (Eu) 2017/745 Of The European Parliament And Council (Or Regulation (Eu) 2017/746 Of The European Parliament And Council), Or In Accordance With The Relevant Transitional Provisions Of These Regulations And Act No. 375/2022 Coll., On In Vitro Medical And Diagnostic Medical Devices, As Amended. New, Unused And Non-Refurbished Goods Must Be Delivered, With A Minimum Warranty Of 24 Months. The Submitted Offer (And Therefore Also The Offer Price) Must Include Delivery, Moving In, Commissioning Of The Goods, Connection To The Clients Data Network With Data Transmission Via The Dicom Standard, Installation Calibration/Validation, Acceptance Tests, Assembly/Installation And Briefing/Training Of Personnel In Accordance With Act No. 375/2022 Coll., On Medical Devices And In Vitro Diagnostic Medical Devices - Hereinafter Also Referred To As On Medical Devices Means. If The Manufacturer Has Limited The Life Of The Offered Goods For Any Reason And In Any Way, This Limitation Must Not Be For A Period Of Less Than 120 Months From Commissioning And Handing Over To The Client. The Goods Must Be Free Of Factual And Legal Defects. The Subject Of The Public Contract Must Meet All The Requirements Imposed On It By The Legal Regulations Of The Czech Republic, As Well As The Requirements Specified In The Tender Documentation.
