1. The Subject Of The Order Is The Design And Implementation Of A Cctv Installation And An Access Control Installation In The Building Of The Provincial Addiction Therapy Center In Gdańsk At Ul. Zakopińska 37, The Scope Of Work Of Which Was Specified In The Functional And Utility Program (Ffu), Constituting Annex No. 1 To The Swz (Basic Order).2.The Ordering Party Applied The Option Right Referred To In Art. 441 Of The Public Procurement Law. The Ordering Party Provides For The Right Of Option In The Form Of Extension Of Construction Works Consisting In Design And Implementation Of Closed-Circuit Television Installation In The Building Of The Provincial Addiction Therapy Center In Gdańsk At Ul. Srebrniki 9. The Use Of The Option Depends On The Ordering Party Obtaining Additional Financial Resources In The Budget For This Purpose. The Ordering Party Will Submit A Declaration Of Exercise Of The Option 14 Days Before The Planned Commencement Of Construction Works In The Second Building, The Scope Of Which Has Been Specified In The Functional And Utility Program (Ffu) Constituting Annex No. 1A To The Swz. The Ordering Party May Exercise The Option Right By October 1, 2026 By Submitting A Written Declaration Of Will To The Contractor. Failure To Submit A Declaration Of Will In The Above-Mentioned Deadline Means Waiving The Option Right In Its Entirety, Without Any Financial Or Legal Claims On The Part Of The Contractor. The Scope Of The Basic Order Is Specified In Chapter I, Section 1. The Requirements Specified In The Swz In Relation To The Basic Contract Shall Apply Accordingly To The Contract Covered By The Option Right. 3. The Subject Of The Order Includes In Particular: 1) Preparation Of Design Documentation, 2) Verification Of The Solutions Provided In The Pfu, 3) Works Related To The Possible Dismantling Of Unnecessary Elements Related To The Above. Installations,4) Installation Of Cctv Installations,5) Installation Of Access Control Installations,6) Installation Of Cctv Installations (Applies To The Order Covered By The Option Right),7) Construction Works Related To The Installation Of The Above-Mentioned Installation,8) Work Related To Electrical Installations For The Installation Of The Above. Installation (Adaptation Of Existing Electrical Installations In Terms Of Overvoltage Protection),9) Delivery Of Specialized Equipment, Including Cameras, Recorders, Access Control Equipment 10) Filling, Sanding And Painting Of Walls In Places Where The Cables Are Routed And The Above-Mentioned Are Installed. Installation In Order To Restore The Condition Before The Commencement Of Construction Works,11) Preparation Of As-Built Documentation.4.The Works Will Be Performed On The Premises Of An Active Facility (Also Applies To The Contract Covered By The Option Law), Which Includes Therapeutic Rooms And Patient Clinics Within The Structures Of The Provincial Addiction Therapy Center.
