Tenders are invited for Provision of Medical Insurance Cover to the State Fertilizer Company Limited for the Year 2026 / 2027. Sealed bids for the continuation of the Medical Insurance Cover at the State Fertilizer Company Limited from 18.11.2026 up to 17.11.2026 will be accepted by the Chairman of the Department Procurement Committee from 03.09.2026 up to 24.09.2026. 02. Only Insurance Companies registered in the Insurance Regulatory Commission can participate for this bid. 03. Relevant specimen forms and conditions for this bid can be obtained from 03.09.2026 up to 23.09.2026 on working days of the week between 9.00 a.m and 3.30 p.m on payment of a non-refundable fee of Rs. 3,500.00 (Three Thousand Five Hundred Only) in cash to the cashier at the Head Office of the State Fertilizer Company Limited at Lakpohora Swarna Jayanthi Mawatha, Hunupitiya, Wattala. 04. Bid forms in duplicate marked as original and duplicate enclosed in separate covers and sealed and said covers enclosed in a single cover and the Bid Number - 2026/27/SFCL/MI/01 mentioned on the top left hand corner of the cover, can be forwarded by registered post to be received at or before 10.30 a.m on 24.09.2026. In the alternative can be deposited in the Bid Box placed at the Procurement Section at the Head Office of the State Fertilizer Company Limited at Lakpohora Swarna Jayanthi Mawatha, Hunupitiya, Wattala. Bids received after stipulated date and time will be rejected. 05. Pre Bid meeting will be held at the Procurement Section of the State Fertilizer Company Limited at 10.30 a.m on 16.09.2026. 06. Bids will be opened immediately after closure of acceptance of bids at 10.30 a.m on 24.09.2026. Bidder or one of his authorized representatives only can be present at the time of bid opening. 07. As mentioned in the bid continuous a refundable Bid Security of Rupees 100,000.00 (One Lakh) should be submitted along with the Bid. For this purpose bank guarantee or original of the receipt obtained for the payment of relevant amount to the Cashier of the State Fertilizer Company Limited can be presented with the bid.
