The Subject Of The Order Is The Delivery Of A Digital X-Ray Ceiling Camera To The Ordering Partys Headquarters In Accordance With The Description And Requirements Contained In Annex No. 1A To The Specification. 2. Offers That Do Not Include The Full Scope Of The Subject Of The Order Will Be Rejected. 3. The Remaining Terms Of The Order Are Specified In The Draft Contract Constituting Annex No. 3 To These Specifications. 4. Cooperation In Connecting The Device To The Pacs System And Purchasing A License On The Contractors Side. The Valuation Of The License Purchase Is Attached In Annex No. 1B To The Specification. Co-Financing Of The Order Under The Project: Construction And Equipment Of A Multi-Specialized Ambulatory Care Team Covering Primary Health Care (Poz) And Specialist Clinics (Aos) At The Ludwik Rydygier Specialist Hospital In Krakow Sp. Z O. O. The Project Is Co-Financed By The Eu Under The European Funds For Małopolska 2021-2027 Program. The Ordering Party Requires The Submission Of The Following With The Offer: A) Ce Certificate Or Declaration Of Conformity; B) Manufacturers Certificates Confirming That The Subject Of The Order Was Manufactured Using A Production Management System Compliant With Directives And/Or Standards Regarding Ecology And Energy Efficiency, Using The Iso 14001 Standard Or Equivalent Issued By An Entity Authorized To Control Quality Or Equivalent, Where By Equivalence The Ordering Party Means The Indication By The Contractor Of Other Certificates Issued By An Independent, Accredited Entity; C) Declaration Of The Contractor Confirming The Compatibility Of The Subject Of The Order With The Provisions Of The Rohs 2011/65/Eu Directive (Restriction Of The Use Of Certain Hazardous Substances In Electrical And Electronic Equipment), Using The Iec 60601-1-9:2020 Standard (Environmental Product Design For Medical Electrical Equipment) Or Equivalent Issued By An Entity Authorized To Control Quality Or Equivalent, Where By Equivalence The Ordering Party Understands The Indicated
