The Subject Of The Order Is The Provision Of Care Services Provided By: - 1 Caregiver, For 5 People Requiring Support In Everyday Functioning Due To Age, Health Condition Or Disability, In Accordance With The Following Detailed Conditions And Requirements: 1) Services Will Be Provided At The Participants Place Of Residence (In The Mniszków Commune) Of The Project By The Caregiver, In The Period From 09/2026 To 07/2028, With The Possibility Of Extension Project Implementation Time.2) The Total Scope Of The Subject Of The Order Includes The Provision Of Care Services In Accordance With The Current Needs Of The Participants And The Schedule Established By The Ordering Party, During The Duration Of The Order.3) The Caregiver Providing Support At The Place Of Residence Will Provide Services For An Average Of 90 Hours Per Month. An Hour Of Service Is A Clock Hour (60 Minutes), Counted As The Actual Time Spent Providing Care Services In The Senior Club. The Services Provided Do Not Include The Time Needed To Reach The Place Of Service Provision.4) The Process Of Providing Services Will Be Ensured 7 Days A Week, With Appropriate Determination Of Working Hours And The Scope Of Tasks Performed By Caregivers, Guaranteeing Their Continuity And Appropriate Quality.5) Payment Of The Contractors Remuneration Will Be Based On The Actual Number Of Hours Of Providing Care Services, Confirmed On The Basis Of Verified Working Time Records.6) The Scope Of Care Services Provided At The Place Of Residence Of The Project Participant Will Include The Activities Indicated In Art. 50 Point 3 Of The Act Of March 12, 2004 On Social Assistance, I.E., Among Others: - Assistance In Meeting Everyday Life Needs, - Support In Hygiene Activities Appropriate To The Form Of Day Care, - Assistance In Care Activities Recommended By A Doctor, To The Extent That Does Not Require Medical Services, - Ensuring, As Far As Possible, Contacts With The Environment.7) The Detailed Scope And Scope Of Services Will Be Determined By The Ordering Party Individually For Each Person Requiring This Type Of Services. Support, Taking Into Account: - The Need To Meet Basic And Necessary Needs, - Other Needs Of The Person Requiring Support, Corresponding To The Goals And Within The Capabilities Of Social Assistance, - The Social, Living And Family Situation Of The Person, His Or Her Health And Psychophysical Fitness, - The Possibility Of Using The Rights And Resources Of The Person And His Or Her Environment (Family, Neighborly Help, Etc.).8) Care Services Are Provided In The Local Community, Respecting The Individual Needs And Capabilities Of Participants.9) The Contractor Is Not Entitled To Reimbursement Of Costs Incurred During The Provision Of A Service, E.G. Purchasing A Meal. The Costs Incurred By The Caregiver Are Not Reimbursed By The Ordering Party.10) The Contractor Is Obliged To Provide Care Services, Taking Into Account The Subjectivity Of The Participants, Including Respecting The Right To Res
