The Subject Of The Order Is The Provision Of Mobile Telephony Services With Mobile Internet And The Delivery Of 23 Mobile Phones And The Delivery Of 37 Sim Cards With Network Access In Lte Technology For The District Office In Mielec.2.The Guaranteed Scope Of The Order Includes:2.1.Package No. 1: A) Delivery Of 21 Sim Cards With Their Activation (Existing Numbers) And The Delivery Of 21 Pieces Of Brand New Telephones (7 Pieces Of Type A Phones, 2 Pieces Of Type B Phones, 12 Pieces Of Type C Phones) And Provision Of The Service Of Access To The Mobile Telephony Network And The Internet In Lte Technology; B) Delivery Of 2 Pieces Of Sim Cards (New Numbers) With The Delivery Of 2 Pieces Of Brand New Phones Telephone Lines (1 Type B Telephone And 1 Type C Telephone) And The Provision Of The Service Of Access To The Mobile Telephone Network And The Internet In Lte Technology;2.2.Package 2: Delivery Of 5 Sim Cards With Their Activation And Provision Of Access To The Mobile Network (Without Telephones);2.3.Package 3: Delivery Of 1 Sim Card And Provision Of Network Access Services With Lte And Gprs Transmission (Without A Telephone Set);2.4.Package 4: Delivery Of 37 Sim Cards And Provision Of Network Access Services In Lte Technology (Without Telephones).3.A Detailed Description Of The Subject Of The Order, Including The Minimum Parameters And Requirements Of The Ordering Party And Important Provisions Of The Contract, Are Set Out In Annex No. 3 To The Swz. 4. If The Subject Of The Order Includes Or May Include An Ict Product, Ict Service Or Ict Process, The Contractor Is Obliged To Offer Only Such Ict Products, Ict Services And Ict Processes Used To Perform The Order, Both As Main Elements And Components, Elements Of The Service Provision Environment Or Accompanying Solutions, Which On The Date Of Submission Of Offers: 4.1.Are Not Included In The Recommendation Referred To In Art. 33 Section 4 Of The Act Of 5 July 2018 On The National Cybersecurity System, Stating Their Negative Impact On The Fundamental Interest Of State Security; 4.2.Do Not Cover An Ict Product Whose Type Was Specified In The Decision To Recognize The Supplier As A High-Risk Supplier Referred To In Article 67B Section 15 Of The Act Of 5 July 2018 On The National Cybersecurity System, Nor The Ict Services Or Ict Processes Specified In This Decision.5.In These Proceedings, In The Opinion Of The Ordering Party, There Are No Activities In The Scope Of Contract Execution That Involve Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code, Therefore The Ordering Party Does Not Determine Pursuant To Art. 95 Section 1 Of The Act, Requirements Regarding The Employment By The Contractor Or Subcontractor Of Persons Performing The Order Under An Employment Contract.
