1. The Subject Of The Order Is The Provision Of Elderly Persons Assistant Services For 6 People From The Świlcza Commune As Part Of The Grant Project Entitled: Innovations In Local Government - Elderly Persons Assistant Implemented Under The European Funds For Social Development Program 2021-2027 Co-Financed By The European Social Fund Plus.2.The Estimated Number Of Clients For Whom Care Services Will Be Provided Every Day On Working Days For A Period Of 8 Months, I.E. No Earlier Than October 1, 2026 And No Later Than May 31, 2027, Is 6. The Expected Number Of Hours Per Month For 6 People Aged 60+ Is 144 Hours, And For 1 Person Per Month - 24 Hours. However, The Estimated Number Of Hours To Be Completed During The Contract Period Is 1,152 Hours In Total.3.The Contractor Is Obliged To Provide Two Assistants To Provide Services For The Elderly Person For 6 People From The Świlcza Commune. Each Of The Two Assistants Will Support Three Project Participants, Providing Them With Individual Help And Support In Everyday Functioning Tailored To Their Needs. It Is Possible To Flexibly Divide The Project Participants Between Two Assistants, Depending On The Individual Needs Of The Participants And Work Organization. Detailed Division Participants Between Assistants May Be Determined In Consultation Between The Persons Providing The Service, Taking Into Account The Provision Of Appropriate And Uninterrupted Support To All Participants.4. The Number Of Hours Of Services And Persons For Whom Care Services Will Be Provided May Change During The Performance Of The Subject Of The Order Due To The Specificity Of The Service, Which Is Characterized By The Variability Of The Needs Of The Clients Due To The Changing Health Condition.5. The Ordering Party Reserves The Right To Make Changes To The Scope Of Services Covered By This Contract, If Necessary Changes In Applicable Legal Regulations.6. The Scope Of The Elderly Persons Assistant Service Includes The Provision Of Care Services For People Aged 60+ In The Scope Of: - Assistance In Shopping For Basic Items, - Assistance In Preparing And Serving Meals, - Assistance In Cleaning The Household, - Assistance In Washing Clothes, - Assistance In Paying Fees, Contacting Doctors, Facilities And Offices, Including Contacting In Official Matters On Behalf Of The Person Covered By The Service, – Accompanying On Walks, – Informing The Family Or Relevant Services About Deterioration Of Health Or Crisis Situations, – Hospital Visits, – Support For The Participant In Moving Outside The Place Of Residence, – Support In Maintaining Social Contacts.7.The Estimated Cost Of One Clock Hour Of Remuneration For The Provision Of Elderly Person Assistant Services Is Pln 55.00 Gross.8. Requirements For The Staff Providing Elderly Person Assistant Services - As Part Of The Performance Of The Subject Of The Order, The Contractor Will Be Obliged To Provide A Minimum Of Two People, I.E.
