Description Of The Subject Of The Order - Implementation Of Specialized Care Services Specialized Care Services - Will Be Provided In The Malanów Commune For Residents Of The Malanów Commune In Their Place Of Residence In The Period From September 1, 2026 To September 30, 2028. The Task Will Be Implemented In Accordance With Article 50 Of The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2026, Item 639) And The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws Of 2024, Item 816). The Scope Of Services Will Be Adapted To The Individual Needs Of People Requiring Assistance In The Form Of Specialized Services. Care, Resulting From The Type Of Their Disease Or Disability, Covering The Following Scope: A) Care As Supporting The Treatment Process, Including: - Assistance In Access To Health Services; - Arranging And Monitoring The Dates Of Medical Visits And Diagnostic Tests; - Assistance In Purchasing/Ordering Medications At The Pharmacy; - Supervising The Intake Of Medications And Observing Possible Side Effects Of Their Use; - In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Auxiliaries And Medical Materials, Items Orthopedics, As Well As In Maintaining Hygiene; - Assistance In Getting To Health Care Facilities; - Assistance In Getting To Rehabilitation Facilities; - Physical Rehabilitation And Improvement Of Disturbed Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004 On Health Care Services Financed From Public Funds; - In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation And Physiotherapy; - Cooperation With Specialists In The Field Of Psychological And Pedagogical Support And Educational And Therapeutic Aiming At Multilateral Activation Of The Person Using Specialist Care Services; The Expected Number Of Hours Of Specialist Care Services During The Period Of Execution Of The Order Is 2,000 Hours: 2026 - 320 Hours; 2027 - 960 Hours; 2028 - 720 Hours. For 10 People Who Need Support For 8 Hours A Month Each. A Total Of 2,000 Hours.
