Hospital Facilities Construction Work – Reconstruction, Modernization Of Hospital Buildings And Retrofitting Of Technical And Medical Infrastructure In Order To Improve The Quality And Access To Comprehensive Psychiatric Care At The Multi-Specialized Provincial Hospital In Gorzów Wielkopolski, Limited Liability Company. The Subject Of The Order Includes The Design And Implementation Of The Reconstruction, Modernization And Adaptation Of Pavilions No. 3, 4, 5, 9, 10 And 17, Along With Installations, Land Development And Preparation Of Facilities For The Installation Of Permanent Equipment Necessary For The Functioning Of Organizational Units. The Implementation Of The Investment, While Maintaining The Historical And Architectural Values Of The Pavilions Entered In The Register Of Monuments And The Historical And Spatial Values Of The Entire Hospital Complex, Is Intended To Improve The Technical Condition And Functionality Of The Facilities, Increase The Availability Of Psychiatric Services And Achieve The Minimum, Measurable Material Effects Indicated In The Pfu. The Implementation Of The Project Should Be Planned In A Sequential Manner, Ensuring Continuity Of Activities Of Organizational Units To The Greatest Possible Extent. The Contractor Is Obliged To Develop An Initial Concept Of Phasing The Works Along With Defining The Necessary Buffer Solutions Enabling The Temporary Transfer Of Medical Functions While Maintaining The Safety Of Patients And Staff. The Scope Of The Order Includes: 1) Development Of A Multi-Sector Program And Spatial Concept; 2) Development Of Complete Design Documentation, Including A Program Of Conservation Works, 3) Obtaining The Required Arrangements, Opinions, Resolutions And Administrative Decisions, Including Decisions On A Building Permit, Permits From The Lubuskie Voivodeship Conservator Of Monuments And Others Required By Law, 4) Performance Of Construction Works Including Installations, 5) Performance Of Conservation Works, If They Result From The Decisions Of The Lubuskie Voivodeship Conservator Of Monuments, 6) Execution Land Development, Including The Implementation Of All Necessary Connections And Reconstruction Of Technical Infrastructure, 7) Delivery, Assembly And Commissioning Of Only Permanent, Built-In, Installation And Technological Equipment And Devices Necessary For The Proper Functioning Of The Facilities, Provided That They Are Indicated In The Fup Or Result From The Approved Design Documentation (Purchase Of Movable Equipment, Movable Furniture, Medical Equipment And Medical Equipment Is Not The Subject Of The Order), 8) Carrying Out Tests, Measurements, Commissioning And Staff Training, 9) Preparation Of As-Built Documentation, 10) Notifying The Competent Authorities About The Completion Of Construction Works And Obtaining An Occupancy Permit.
