I. The Subject Of The Order Is The Ongoing Provision Of Comprehensive Funeral Services For The Social Welfare Center Of The Ochota District Of The Capital City Of Warsaw On The Terms Set Out In The Annex To Resolution No. Lx/1840/2009 Of The Capital City Council. Warsaw Of August 27, 2009, As Amended. Ii. Services Include:1. Completing All Formalities Related To Death And Funeral On Behalf Of The Ordering Party (Receiving A Written Order From The Ordering Party, Obtaining A Death Card And Death Certificate, Completing Formalities Related To The Purchase Of A Burial Place By Ops, Paying Fees For Retaining Rights To The Grave, Etc.).2. Collection Of Bodies From The Place Indicated By The Ordering Party And Their Transport Within Warsaw And To The Cemetery.3. Storing The Body In A Cold Room (If Necessary).4. Purchase And Transport Of The Cheapest Pine Coffin With Basic Accessories.5. Preparation Of The Body For Burial.6. Putting The Body In The Coffin.7. Transport Of The Coffin Within The Cemetery.8. Burial Of The Body In A Single Grave.9. Purchase And Installation Of A Tombstone.10. Purchase And Installation Of A Religious Symbol.11. Providing Partial Liturgical Service (Taking People From The Cemetery Gate And Praying At The Grave) In Accordance With The Confession Of The Deceased.12. Burial Of People Who Died Of Infectious Diseases, Human Remains, Etc. Iii. Burial Place - The Main Burial Place Will Be The Southern Municipal Cemetery In Antoninów.
