1. The Subject Of The Order Is The Provision Of A Service Consisting In Comprehensive Investor Supervision During The Implementation Of The Investment Titled: Construction Of A Pocket Park At Ul. Warsaw Uprising In Koło Implemented In The Design-Build System.2. The Investment Is Located In Koło, Plot No. 11/1, 12, 13/1, 41, 42, Map Sheet 34.3. General Data Of The Subject Of The Order: 1) The Investment Includes In Particular Works Related To The Revitalization Of The City Park, Including: A) Preparation Of Design Documentation, Obtaining All Required Opinions, Arrangements And Permits, B) Construction And Construction Works, C) Execution And Maintenance Of Green Areas, Including Planting Trees, Shrubs And Ornamental Plants, D) Elements Of Small Architecture And Accompanying Infrastructure.2) Detailed Scope Of Works Covered By Investment Is Specified In The Functional And Utility Program Constituting An Annex To The Swz. 3) The Ordering Party Requires That The Investors Supervision Be Carried Out By A Person/Persons Having: A) Construction Qualifications To Perform Independent Technical Functions In The Construction Industry In The Field Of Construction, Without Restrictions Or To The Extent Enabling The Implementation Of The Subject Of The Order, And A Current Entry On The List Of Members Of The Relevant Chamber Of Professional Self-Government Of The Polish Chamber Of Civil Engineers, Confirmed By A Current Certificate Of Membership In The Chamber. Higher Education Required, At Least 10 Years Of Professional Experience, Including 5 Years Of Experience In Supervising Construction Works. B) Construction Qualifications To Perform Independent Technical Functions In The Road Specialty And A Current Entry On The List Of Members Of The Relevant Chamber Of Professional Self-Government Of The Polish Chamber Of Construction Engineers, Confirmed By A Current Certificate Of Membership In The Chamber. Higher Education Required, At Least 10 Years Of Professional Experience, Including 5 Years Of Experience In Supervising Road Works. C) Qualifications And Experience In The Field Of Greenery. Required Higher Education In Horticulture Or Landscape Architecture And Min. 3 Years Of Work Experience In Managing Or Supervising The Planting Of Greenery Or Secondary Education In Horticulture Or Landscape Architecture And Min. 5 Years Of Experience In Managing Or Supervising The Implementation Of Greenery Plantings.4) The Basic Duties Of The Contractor Will Include In Particular: A) Design Stage: - Representing The Ordering Party By Checking The Compliance Of The Design Documentation With The Functional And Utility Program And The Investors Guidelines; - Checking The Completeness Of The Design Documentation, Giving Opinions On Its Implementation In Accordance With The Fup, Applicable Regulations And The Contract Concluded With The Contractor; - Participation In Meetings And Conferences Regarding The Design Stage. B) Investment Implementation Stage: - Participation In Hand
