The Subject Of The Order Is The Supply Of Computer Equipment For The Municipal Office In Wałbrzych, In Accordance With Annex No. 6 To The Swz Constituting The Description Of The Subject Of The Order. 2. Information About The Evidence Confirming That The Offered Supplies Meet The Requirements Specified By The Ordering Party. In Order To Confirm That The Deliveries Meet The Requirements Specified By The Ordering Party, The Ordering Party Requests That The Contractor, Along With The Offer, Submit A Declaration Of Compliance Of The Subject Of The Offer With The Description Of The Subject Of The Order. In The Declaration On The Compliance Of The Subject Of The Offer With The Description Of The Subject Of The Order (Appendix 1A To The Swz), The Contractor Will Provide The Manufacturer, Model And Country Of Origin Of The Offered Delivery Parts. The Ordering Party Informs That If The Evidence Is Not Submitted Or If The Evidence Submitted Is Incomplete, The Ordering Party Will Call On The Contractor To Submit Or Supplement It. 3. The Subject Of The Order Includes Ict Products, Ict Services And Ict Processes, Listed In Annex No. 3 To The Swz. The Ordering Party Requires That The Offered Service, Including All Ict Products, Ict Services And Ict Processes Used To Complete The Order, Both As Main Elements And As Components, Elements Of The Service Provision Environment Or Accompanying Solutions, Does Not Include: 1) Ict Products, Ict Services Or Ict Processes Indicated In The Recommendation Referred To In Art. 33 Section 4 Of The Act Of 5 July 2018 On The National Cybersecurity System, Stating Their Negative Impact On The Fundamental Interest Of State Security, 2) An Ict Product Whose Impact Was Determined In The Decision To Recognize The Supplier As A High-Risk Supplier, Referred To In Art. 67B Section 15 Of The Act Of 5 July 2018 On The National Cybersecurity System Nor The Ict Services Or Ict Processes Specified In This Decision. Failure To Meet The Above Requirement Results In Rejection Of The Offer Pursuant To Art. 226 Section 1 Point 17 And Point 19 Of The Public Procurement Law. Before Selecting An Offer, The Ordering Party Will Each Time Verify Whether A Decision Covering The Product, Service Or Ict Process That Is The Subject Of The Contractors Offer Has Been Published. Examination Of Offers In The Scope Of The Conditions Specified In Art. 226 Section 1 Point 19 Of The Public Procurement Law Will Also Be Subject To Verification Of The List Of Entities Against Which A Decision Was Issued By The Minister Responsible For Computerization Pursuant To Art. 67B Section 15 Of The Act Of 5 July 2018 On The National Cybersecurity System. This Means That, In Accordance With The Above-Mentioned Regulations, The Minister Responsible For Computerization Will, By Way Of A Decision, Recognize The Supplier Of Equipment Or Software And Entities Belonging To The Capital Group, Within The Meaning Of Art. 3 Section 1 Point 44 Of The Act Of 29 September 1994 On Accounting, Within Which The Supplier Operates, As A High-Risk Supplier If This Supplier Poses A Threat To The Fundamental Interest Of State Security.
