The Subject Of The Order Is The Purchase, Delivery, Installation And Commissioning As Well As Training Of The Ordering Partys Employees In The Operation Of A Flow Analyzer For Cyanide Determination Along With The Equipment, Hereinafter Referred To As The Analyzer Or Equipment In The Swz, To The Registered Office Of The Ordering Party Indicated In The Description Of The Subject Of The Order, I.E.: Provincial Sanitary And Epidemiological Station In Kraków, Ul. Prądnicka 76, 31-202 Kraków, On Business Days And Hours, I.E. From Monday To Friday From 7:30 A.M. To 3:00 P.M. 2. The Subject And Scope Of The Order Has Been Described In Detail: A) In Annex A To The Swz - Technical Parameters And Requirements; B) In The Contract Template Constituting Annex 2 To The Swz. 3. The Ordering Party Requires That The Offered Service, Including All Ict Products, Ict Services And Ict Processes Used To Perform The Order, Both As Main Elements And Components, Elements Of The Service Provision Environment Or Accompanying Solutions, Does Not Include: - Ict Products, Ict Services Or Ict Processes Indicated In The Recommendation Referred To In Art. 33 Section 4 Of The Act Of July 5, 2018 On The National Cybersecurity System, Stating Their Negative Impact On The Fundamental Interest Of State Security (Consolidated Text: Journal Of Laws Of 2026, Items 20 And 252); - An Ict Product Whose Type Was Specified In The Decision To Recognize The Supplier As A High-Risk Supplier Referred To In Art. 67B Section 15 Of The Act Of 5 July 2018 On The National Cybersecurity System, Nor The Ict Services Or Ict Processes Specified In This Decision. Failure To Meet The Above Requirement Results In Rejection Of The Offer Pursuant To Art. 226 Section 1 Point 17 Or Point 19 Of The Public Procurement Law. 4. To Confirm That The Offered Equipment Meets The Requirements Of The Ordering Party And The Requirements Specified In Section 3, A Completed Annex A To The Swz Must Be Submitted Along With The Offer - Technical Parameters And Requirements (Including The List Of Ict Elements Used During The Execution Of The Order Completed By The Contractor To Identify Ict Products, Ict Services And Ict Processes Covered By The Offer Or Used To Execute The Order), Signed With A Qualified Electronic Signature By A Person Authorized To Represent The Contractor.
