Part One Of The Order: • 832 Diagnoses In The Form Of Consultations With A Speech Therapy Specialist, A Total Of 832 Hours Of Consultations. It Is Assumed That A Child Attending The Preschool Education Center In The Bof Area Will Receive, As Part Of This Part Of The Order, A Diagnosis In The Form Of A Consultation With A Speech Therapy Specialist, Including 1 Hour Of Consultation, Along With Written Feedback For The Parent Containing At Least The Following Data: Contractors Header Stamp, Name And Surname Child, Date Of Diagnosis In The Form Of Consultation With A Specialist, Description Of The Identified And/Or Reported Problem, Recommendations For Further Work With The Child And/Or Recommendations For Further Diagnostics, Signature Of The Speech Therapy Specialist Conducting The Consultation And Preparing The Written Feedback. The Condition For The Provision Of The Service Is Specific To The First Part Of The Order. The Contractor Will Provide At Least Two Specialists: Each Of Whom Has Completed Higher Education In The Field Of Speech Therapy, In Particular A Uniform Masters Degree In Speech Therapy Or Postgraduate Studies In Speech Therapy Or Neurological Speech Therapy, Completed After Obtaining Higher Education; Are Not Included In The Register Referred To In The Act Of May 13, 2016 On Counteracting Threats Of Sexual Crime (Journal Of Laws Of 2026, Item 110, As Amended); Verified In The Register Of Sexual Offense Perpetrators And Having A Certificate From The National Criminal Register.
