1. The Subject Of This Order Are Construction Works Related To The Implementation Of The Task Titled: Replacement Of Window Joinery In The Main Building Of The Industrial History Museum In Opatówek.2. The Detailed Scope Of Works Is Specified In The Offer Cost Estimate (Appendix No. 1 To The Swz) And Design Documentation (Appendix No. 8 To The Swz).3. The Subject Of The Order Includes The Implementation Of Construction Works In The Main Building Of The Museum Of The History Of Industry In Opatówek, Which Is An Immovable Monument Entered In The Register Of Monuments Kept By The Provincial Conservator Of Monuments, Regarding, Among Others: 1) Forging Steel Frames Or Window Grates From The Wall, 2) Replacing Windows, 3) Applying Plaster On The Jambs. As Part Of The Order, The Contractor, Before Replacing The Windows, Will Present Prototypes Of The Planned To Install Windows For Assessment By A Conservation Commission Composed Of Representatives Of: The Ordering Party, The Contractor And The Provincial Conservator Of Monuments.4. It Is Recommended That Contractors Inspect The Building That Is The Subject Of This Order And Its Surroundings In Order To Evaluate The Documents And Information Provided By The Ordering Party As Part Of This Procedure And To Obtain Any Information That May Be Necessary In Preparing The Offer. The Ordering Party Informs That The Building In Which Construction Works Will Be Carried Out Will Remain In Use Throughout The Duration Of The Order. The Contractor Is Obliged To Organize And Carry Out The Works In A Way That Does Not Disrupt The Current Functioning Of The Museum And Ensures Its Uninterrupted Use Throughout The Entire Period Of Contract Execution. 5. The Ordering Partys Requirements Regarding The Employment Of Persons Under An Employment Contract By The Contractor Or Subcontractor. 1) The Ordering Party, Pursuant To Art. 95 Section 1 Of The Public Procurement Act Requires The Contractor Or Subcontractor To Employ, Under An Employment Contract, Persons Performing Activities Related To The Execution Of The Order, The Performance Of Which Consists In Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code (Journal Of Laws Of 2025, Item 277, As Amended). This Requirement Applies To Persons Who Perform Activities Directly Related To The Performance Of The Works, I.E. The So-Called Manual Workers.2) The Contractor Is Obliged To Include In Each Subcontracting Agreement Appropriate Provisions Obliging Subcontractors To Employ Under An Employment Contract All Persons Performing The Above-Mentioned Activities.3) Within 14 Days From The Conclusion Of The Contract, The Contractor Is Obliged To Submit To The Ordering Party A Declaration That The Persons Performing The Activities Specified In Point 1 Are Employed On The Basis Of An Employment Contract Within The Meaning Of The Provisions Of The Act Of June 26, 1974, Labor Code (Journal Of Laws Of 2025, Item 277, As Amended) Along With Showing, A
