The Subject Of The Order Includes In Particular: 1. Construction Of A Water Supply Network With A Total Length Of Approximately 5.55 Km Dz 560 Pe100rc And Dn500 Cast Iron, Including: 1) As Part Of The Task Entitled Construction Of A Strategic Transit Water Pipeline From Lake Bystrzyckie (Water Treated In Suw Lubachów) To The Communes Of The Dzierżoniów District - Stage Iv: Water Supply Network With A Length Of Approximately 0.52 Km Within The Village Of Lubachów, Covering The Section From The W21 Junction To The W47 Junction; 2) As Part Of The Task Entitled Replacement Of Section I Of The Lubachów Water Pipeline: A Water Supply Network With A Length Of Approximately 5.03 Km On The Section From The Town Of Lubachów, Through Zagórze Śląskie, Bystrzyca Górna, Burkatów To Bojanice - From The W1 Node To The W21 Node And From The W47 Node To The W173 Node, Along With A Surge Node To The Existing Dn600 Cast Iron Water Pipeline (Within Stage I Lubachów Water Supply: Node W1 With Air Vents And Gate Valves), Excluding: - The Already Completed Section Of The Water Supply Network Between Nodes W94-W96. Air Vents Odp.12 And Odp.13 Will Be Made By The Contractor In The Location Agreed With The Ordering Party; - The Section Of The Water Supply Network Currently Under Construction From The Odp.6 Vent (Without It) To The Border Of Plot No. 76, Lubachów District, Behind The W64 Junction; - The Section Of The Water Supply Network Currently Under Construction From The W69 Junction Through The W70 Junction. Towards The W71 Junction 2. Reconstruction Of The Road Surface In Accordance With The Current Conditions Of Road Managers - Attached To Part Iii Of Swz - Opz. 3. The Scope Of The Task Also Includes The Preparation Of A Set Of Documents Constituting Attachments To The Application For An Occupancy Permit / Notification Of Completion Of Construction Works, Including In Particular: A) A Statement From The Construction Manager, B) Necessary Research And Verification Protocols (Including Soil Compaction Tests, Tightness Tests), C) As-Built Inventory, Along With Information From A Surveyor Regarding The Compliance Of The Building With The Plot Or Area Development Plan. D) Copies Of The Drawings Included In The Approved Construction Design With Changes Made And Supplemented With A Description (Required In The Case Of Insignificant Changes In Relation To The Approved Design Or The Conditions Of The Building Permit), E) Declarations Of The Authorities Mentioned In Art. 56 Of The Construction Law. 4. Due To The Time That Has Passed Since The Development Of Design Documentation And Obtaining Building Permits, The Ordering Party Informs That The Development And/Or Development Of The Area May Have Changed. 5. The Ordering Party Points Out That The Actual Hydrogeological Conditions (In Particular In Terms Of Soil Workability Categories And Water Table Levels) May Differ (Also Unfavorably) From The Hydrogeological Conditions Identified Through Point Research Wells. 6. The Scope Of The Order Also Includes Surveying, Geological And Archaeological Services. 7. In Order To Meet The Requirements Specified In The Letter Of The Regional Director For Environmental Protection In Wrocław No. Wpn.6335.354.2025.Pt Of November 12, 2025, The Contractor Will Ensure Environmental Supervision
