A Detailed Description Of The Subject Of The Order Is Included In The Description Of The Subject Of The Order, I.E. Annex No. 2 To The Swz. The Quantities Given In The Assortment And Price Form Refer To A Period Of 6 Months (A Contract Will Be Concluded For Such A Period), These Quantities Are Indicative And May Change Depending On The Needs Of The Hospital. The Ordering Party Allows For Changes In Product Quantities In A Given Item Within The Scope Of The Value Of The Package For Which The Contract Will Be Concluded. 2. The Contractor Undertakes To Deliver The Subject Of The Contract Using Its Own Transport Or Through A Professional Courier Company, At Its Own Expense And Risk, To The Headquarters Of: 1) Apteka Wielo Specialist Szpital Powiatowy S.A., Ul. Pyskowicka 47-51, 42-612 Tarnowskie Góry Or Another Facility Indicated By The Ordering Party - Applies To Items No. 1.1, 1.2 2) To The Hospital Pharmacy Department Or Another Place Indicated In The Order At The Ordering Partys Headquarters - Applies To Items No. 1.3, 1.4 - Where It Will Be Unloaded And Handed Over To Authorized Persons - With The Proviso That The Safety Of The Contractors Employee Performing His Duties Will Be Ensured Among Others By Avoiding Close Contacts With Medical Staff And Patients, In Particular By Ensuring And Maintaining Separate Access Routes/Contacts Intended Separately For Servicing Patients And Separately For Handling Deliveries Of Medicinal Products By Employees Of Pharmaceutical Wholesalers.
