3.1.1. Providing Respite Care Services (By 6 Caregivers) To Family Members Or Guardians Providing Direct Care For 10 Disabled People Who Have: 1) A Certificate Of A Significant Degree Of Disability Or 2) A Certificate Treated On An Equal Footing With The Certificate Mentioned In Point 1. From The Area Of The Łabunie Commune. Respite Care Involves Relieving The Burden Of Family Members Or Guardians Of Disabled People (Listed In Point 3.1.2. Below) By Supporting Them In Daily Duties Or Providing Temporary Replacement By The Contractors Guardians.3.1.2. The Estimated Total Maximum Amount Of Respite Care Services To Be Provided Is 1,000 Clock Hours (Including) For 10 Disabled People With: 1) A Certificate Of A Significant Degree Of Disability Or 2) A Certificate Treated On An Equal Footing With The Certificate Mentioned In Point 1 From The Area Of ​​The Łabunie Commune. 3.1.3. Additional Conditions For The Implementation Of The Subject Of The Order: 1. The Service Must Be Provided In An Effective, Economical, Timely Manner, With The Utmost Care, In Accordance With Applicable Standards And Legal Regulations And The Program (Constituting Annex No. 7 To The Swz).2. The Task Will Be Carried Out From Monday To Sunday, Depending On The Reported Needs, Within The Hours And Scope Specified In The Application Form For The Respite Care Program For Local Government Units - 2026 Edition - Annex No. 8. The Maximum Length Of Uninterrupted Provision Of Respite Care Services In The Form Of A Daily Stay At The Place Of Residence May Not Exceed 12 Hours For One Disabled Person. 3. The Parties Agree That The Scope Of Respite Care Services, Depending On The Individual Needs Of A Disabled Person, Includes: A) Assistance In Meeting Everyday Life Needs B) Care Activities - Assistance In Meeting Physiological Needs: Taking To The Toilet, Putting On And Changing Absorbent Materials, Preventing The Formation Of Bedsores And Sores, Taking Care Of Personal Hygiene, Including: Washing, Combing, Shaving, Changing Bed Linen And Underwear, Dressing.C) Nutritional Activities: Preparing Or Assisting In The Preparation Of Meals (Including One Hot Meal), Delivering Ready-Made Meals, Feeding Peg.D) Household Activities: Bringing Fuel, Burning The Stove, Washing Clothes, Shopping, Cleaning.E) Reporting Medical Appointments And Laboratory Tests, Filling Prescriptions,F) Taking Care Of Taking Medications In Accordance With The Doctors Recommendations,G) Mobilizing The Patient To Actively Spend Free Time And Organizing It. 4. As Part Of The Task, The Number Of Hours Of Respite Care Provided For 10 Participants Cannot Exceed 100 Hours For 1 Caregiver Or Family Member Of A Disabled Person;
