1. The Subject Of The Order Is A Transport Service For Children From The Commune Of The City Of Czerwionka-Leszczyny To And From School In The School Year 2026/2027, I.E. On The Route Of The Commune And The City Of Czerwionka-Leszczyny: Czerwionka - Chorzów District In The Period From September 1, 2026 To June 25, 2027 (A Total Of 188 Days Of School Classes). In The Event Of Temporary Closure Of Educational Institutions In The 2026/2027 School Year Due To E.G. Dąbrowskiego - Ul. Hajducka (2Nd Stop: Ul. Hajducka 22, Special Educational Center For Blind And Visually Impaired Youth In Chorzów). Return Route: Route As Above In The Opposite Direction.4. Number Of Transported Children: 2Note: The Ordering Party Will Provide The Exact Addresses Of The Stops And Personal Data Of The Transported Children To The Selected Contractor After Signing The Contract For The Execution Of The Order And The Contract For Entrusting The Processing Of Personal Data.5. The Contractor Is Obliged To Provide Care For Transported Children, Place Children Safely In The Vehicle And Ensure Safety When Entering And Exiting The Vehicle. For This Purpose, He Must Provide, At His Own Expense, At Least 1 (One) Guardian Of Children For The Entire Round Trip Route, Trained In Pre-Medical Assistance Or Holding A Diploma Of A Professional Nurse/Vocational Nurse.
