The Order Will Be Implemented On The Basis Of: - Granted Decision No. 157/2025 Of June 6, 2025 (Final Decision) For The Construction Permit For The Project Titled Expansion And Reconstruction Of The Building Of The Świętokrzyskie Hospital Center Of Pediatrics (Building Registry No. 2437) With A Provincial Childrens Clinic As Part Of The Investment Strengthening Outpatient Specialist Care In The Provincial Hospital Complex In Kielce, On Plots Registry No. 389/10, 389/11, 389/12, 389/13, District 0015, In The Area Of Ul. Grunwaldzka 45 And Ul. President Stefan Artwiński In Kielce - Multi-Discipline Design Documentation For The Expansion And Reconstruction Of The Building Of The Hospital Of The Świętokrzyskie Center Of Pediatrics (Building Registration No. 2437) With A Provincial Childrens Clinic As Part Of The Investment Strengthening Outpatient Specialist Care In The Provincial Combined Hospital In Kielce, On Plots No. 389/10, 389/11, 389/12, 389/13, Precinct 0015, In The Area Of Ul. Grunwaldzka 45 And Ul. President Stefan Artwiński In Kielce By 4 Ideaa Karol Sitarski, Ul. Złota 15/U5, 25-015 Kielce. As Part Of The Expansion And Reconstruction, It Is Planned To Add A Childrens Clinic To The Existing Building Of The Pediatric Hospital. The Planned Extension Is A 3-Story Segment, Partly With A Basement, Connected To The Main Building Of The Hospital By A Connector, Located On The First Floor (Above The Ambulance Driveway), With A Volume Of Approximately 5,500.00 M3, Including The Necessary Installations And Development Of The External Area. The Planned Childrens Clinic Will Be A Health Care Facility Providing Outpatient Health Services To People Under 18 Years Of Age And Will Be Included In The Aos (Outpatient Specialist Care) Clinic With 15 Specialist Clinics. The Subject Of The Contract Is Divided Into The Following Stages: A) Stage I - Execution Of All Construction Works Consisting In The Expansion And Reconstruction Of The Building Of The Świętokrzyski Hospital, Pediatric Center, With A Hospital Clinic For Children Along With The Necessary Accompanying Infrastructure, Land Development, Construction Of Networks And Non-Dental Installations For The Functioning Of The Subject Of The Contract Based On The Design Documentation Of The Task And Preparation Of Comprehensive As-Built Documentation Of The Task Within 10 Months From The Date Of Handing Over The Construction Site. B) Stage Ii - Obtaining A Valid Occupancy Permit For The Ordering Party Within 3 Months From The Date Of Signing The Final Acceptance Of The Subject Of The Contract. C) Stage Iii - Removal Of Defects And Faults In The Subject Of The Contract And Provision Of Maintenance Services For Installed Systems And Devices During The Warranty/Construction Warranty Period. If The Contractor Proposes A Shorter Order Completion Date In The Criterion For The Completion Date Of Stage I Of The Order, He Will Be Obliged To Carry Out Construction Works Within The Proposed Deadline Under The Contract. Amount Of Security: The Amount Of Security For Proper Performance Of The Contract Is 3% Of The Gross Contract Value.
