I. The Subject Of The Order Is: Delivery Of A Modern, Brand New, Unused, Complete, Free From Physical (Construction) And Legal Defects, Based On The Procedure Entitled: Delivery Of A Panoramic X-Ray For A Specialist Clinic In Tarnobrzeg. The Ordering Party Requires The Delivery Of Equipment Manufactured After January 1, 2025. It Is Not Allowed To Offer Reconditioned Or Demonstration Medical Equipment. The Delivered Equipment Must Be Complete And Ready For Use Without Additional Purchases. A Detailed Description Of How To Implement The Subject Of The Contract Is Specified In Annex No. 1A To The Swz And The Draft Contract Constituting An Integral Part Of The Swz. I. Description Of The Subject Of The Order 1. Main Cpv Code According To The Common Procurement Vocabulary: 33124200-7 - Radiodiagnostic Devices Additional Cpv Code According To The Common Procurement Vocabulary: 33111500-6 (Dental X-Ray Machines). 2. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 1 To The Swz - Description Of The Subject Of The Order. 1) The Contractor Is Obliged To Complete All Items In Annex No. 1 And 3 To The Swz. 2) The Delivery Includes: Purchase Of Medical Equipment - Panoramic X-Ray Scanner - Hereinafter Referred To As Equipment; A) Assembly/Installation, Commissioning And Testing Of The Above-Mentioned Equipment; B) Training Of Designated Employees Of The Ordering Party In The Use Of The Above-Mentioned Equipment; C) Training Of Technical Staff In The Field Of Maintenance And Service Of The Above-Mentioned Equipment; D) Warranty Service Of The Above-Mentioned Device, Under The Terms Specified In § 8 Of The Draft Agreement; E) Providing A Warranty Card; F) Providing A Technical Passport For The Above-Mentioned Equipment, G) Providing An Operating Manual For The Medical Device In A Paper Version In Polish, In An Electronic Version In Polish And Technical (Service) Documentation. Ii. The Medical Devices That Are The Subject Of This Agreement Must Meet The Requirements Of The Current Legal Status, In Particular Those Introduced By The Act Of April 7, 2022 On Medical Devices (Journal Of Laws 2024. 1620, As Amended). Iii. The Parameters Of The Standard Indicated By The Ordering Party Represent The Technical, Operational, Utility, Functional Conditions And Other Features Relevant To The Subject Of The Order, While The Brand Or Trade Name Indicated Determines The Class Of The Product, Not A Specific Manufacturer. The Contractor May Offer Equivalent Products, Provided That They Have Technical Parameters No Worse Than Those Described And Given In The Swz. Iv. The Contractor Is Responsible For Quality And Compliance With The Technical And Quality Conditions Described For The Subject Of The Order. V. The Rules For The Implementation Of The Order In Question Are Specified In The Draft Contract Constituting Annex No. 7 To The Swz.
