The Subject Of This Order Is The Purchase, Delivery, Installation And Commissioning Of A Device For Transcranial Magnetic Stimulation, Together With The Training Of Certified Personnel Confirmed By An Appropriate Document (Minimum 10 People) - Indicated By The Ordering Party In The Scope Of Proper, Safe And Effective Operation Of The Offered Device (Hereinafter Referred To As: Subject Of The Order).2. The Subject Of The Order Covered By This Procedure Received Co-Financing From The Budget Of The Masovian Voivodeship As Part Of The Implementation Of The Task Entitled: Purchase Of Equipment For Magnetic Stimulation For The Provincial Independent Complex Of Public Health Care Facilities. The Subject Of The Order Referred To In Section 1 Of This Chapter Must Be: 1) Brand New; 2) Produced No Earlier Than In 2026, 3 Not Previously The Subject Of Exhibitions; 2) Free From Third Party Encumbrances; 3) Originally Packed, Showing No Signs Of Use; 4) Undamaged (Except Where Necessary Assembly Or Installation); 5) Professionally Made, Maintaining High Quality, Aesthetics And Durability; 6) Be Delivered Complete And Intact, Once Installed, Ready To Work As Intended, Without Any Additional Purchases; 7) A Medical Device Within The Meaning Of The Act Of April 7, 2022 On Medical Devices (Journal Of Laws Of 2024, Item 1620); 8) Meet The Requirements Of Relevant Standards And Regulations, In Particular Those Specified In The Act Of April 7, 2022 On Medical Devices (Journal Of Laws Of 2024, Item 1620); 9) Have Appropriate Approvals, In Particular The Ce Certificate, Quality And Approval Certificates And Other Documents Confirming That It Meets All Standards And Requirements Provided For By Law; 10) Have The Manufacturers Declaration Of Conformity (Applies To All Classes Of Medical Device); 11) Authorized For Marketing In The Republic Of Poland And Have A Notification To The Office For Registration Of Medicinal Products, Medical Devices And Biocidal Products; 12) Consistent With The Technical Parameters Specified In The Description Of The Subject Of The Order Constituting Annex No. 1A To The Swz/Annex No. 2 To The Contract (An Integral Part Of The Contract).
