1. The Subject Of The Order Is The Supply Of Cleaning Products In Accordance With The Description Of The Subject Of The Order Included In The Assortment And Price Form, Constituting Annex No. 5 To The Swz And Annex No. 1 To The Draft Provisions Of The Contract.2. The Subject Of The Order And The Method Of Execution Of The Order Include The Draft Provisions Of The Contract (Including Annexes). 3. Pursuant To Article 441 Of The Act Of September 11, 2019. Public Procurement Law, The Ordering Party Provides For The Possibility Of Exercising The Option Right. The Ordering Party Has Determined The Basic Level Of The Order, Which Can Be Expanded By Increasing The Quantity Of The Product To The Maximum Quantity Specified In The Table Of The Product And Price Form In The Column Intended For This Purpose (Optional Quantity). The Ordering Party Stipulates That Deliveries Covered By The Option Right Must Be Made Under The Conditions Specified In Detail In The Draft Provisions Of The Contract.4. The Contractor Is Obliged To Provide, Together With The First Delivery: A) Current Safety Data Sheets In Polish For All Chemical Products Specified By The Ordering Party In Annex No. 5 To The Swz - If Required In Accordance With The Description Of The Subject Of The Order B) Notification To The National Information System On Cosmetics In Accordance With The Act On Cosmetics Of March 30, 2001. (Journal Of Laws No. 42, Item 473, Consolidated Text) And The Regulation Of The Minister Of Health Of May 25, 2004 (Journal Of Laws No. 138, Item 1471) - If Required In Accordance With The Description Of The Subject Of The Order. C) Declarations Of Conformity, Ce Certificates - If Required In Accordance With The Description Of The Subject Of The Order.4.1.
