Other Community, Social And Personal Services – Provision Of Health And Home Care For Fredensborg Municipality. The Offer Includes The Provision Of Comprehensive Care According To The Elderly Act As Well As Practical Help And Support And Personal Help And Care According To The Service Act, As Well As Delegated Health Care Services For Citizens Living At Home In Fredensborg Municipality. The Tender Is Carried Out In Accordance With Section Iii Of The Public Procurement Act, Cf. Lbk No. 10 Of 06/01/2023 With Later Amendments, Section Iii §§ 186-189, Which Relate To Procurement Under The So-Called Light Regime Of Social And Other Specific Services Above The Threshold Value. According To The Elderly Act And The Service Act, Citizens Have The Right To Choose Between At Least Two Suppliers Of Practical Help And Personal Care. The Client Wishes To Offer Citizens The Choice Between One Private Supplier And Municipal Home Care. The Ordering Party Does Not Issue Certificates Of Free Choice. The Supplier Is Found Via The Tender Model. The Tendering Model Implies That The Ordering Party Enters Into One Framework Agreement, Where The External Supplier Delivers One Price, And That The Municipal Supplier Delivers At A Different Price. The Benefits Include Personal Help And Care, Support For Necessary Practical Tasks In The Home As Well As Rehabilitation In The Event Of Functional Impairment Not Related To Hospitalization, Cf. § 10 Of The Elderly Act, As Well As §§ 5, 9 And 11. In Addition, The Services Are Included In Accordance With § 83, § 83A And § 84 Of The Service Act, As Well As Delegated Healthcare Services In Accordance With § 138 Of The Health Act, With The Further Adjustments As Follows, According To The Extent Of The Health Reform Will Be Fully Recognized At The Beginning Of 2027. The Framework Agreement Thus Covers Both Citizens Over 67 And Citizens Under 65 Who Are Inspected By The Center For The Elderly And Prevention. For A More Detailed Description Of The Services Offered, As Well As Requirements For The Services That The Tenderer Must Meet, Please Refer To The Framework Agreement With All Associated Annexes. The Ordering Party Wishes To Enter Into An Agreement With An Ordinary Agreement Period Of 4 (Four) Years, With The Possibility Of Extension(S) Of 2 X 24 (Twenty-Four) Months In Total, In Addition To The Stated Ordinary Agreement Period. This Will Mean A Total Agreement Period Of A Total Of Up To 8 (Eight) Years. With The Offered Framework Agreement, Fredensborg Municipality Wishes To Support A Professional And Constructive Collaboration Between The Ordering Party And The Supplier (Hereafter The Supplier) Throughout The Agreement Period. The Ordering Party Will Continuously Monitor That The Supplier Fulfills The Agreed Conditions. The Supplier Must Be Particularly Willing To Contribute To Creating A Constructive Collaboration With All Relevant Professional Areas At The Ordering Party. The Follow-Up Will Take Place On A Daily Basis, At Interdisciplinary Meetings, But Also At Status Meetings, Where Follow-Up Is Made On Whether The Terms Of The Framework Agreement Are Being Fulfilled And On How The Collaboration Works. As An Element In Ensuring A Good And Long-Term Cooperation Between The Municipality And The Supplier, The Framework Agreement Contains A Penalty, Which Is Defined And Structured With A View To Supporting The Cooperation Throughout The Agreement Period.
