5.1 Name Of The Order: Construction Of A Parking Lot At Primary School No. 5 In Andrychów. 5.2 The Subject Of The Order Includes In Particular: Construction Of A Parking Lot At Primary School No. 5 In Andrychów - Including The Construction Of A Maneuvering Road, Sidewalks, Field Stairs, 48 Parking Spaces, Elements Of The Storm Sewage Network, Demolition And Construction Of The Power Grid, Reconstruction Of Sidewalks, Demolition And Construction Of The Telecommunications And Power Grid (Lv And Mv). 5.3 The Description Of The Subject Of The Order Is Included In The Annexes To The Swz, I.E.: Annex No. 3 And 3.1 Design Documentation, 3.2 Technical Specification For The Execution And Acceptance Of Works, 3.3 Bill Of Quantities. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Draft Contract. 5.4 The Ordering Party Requires Pursuant To Art. 95 Section 1 Of The Public Procurement Law, Employment Under An Employment Contract In Ones Own Enterprise Or By A Subcontractor Of Persons Performing Activities Specified By The Ordering Party In The Scope Of Contract Execution, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code, I.E.: Physical Work On The Construction Site, Including: Preparatory And Demolition Works, Earthworks, Regulation And Protection Of Underground Networks, Drainage, Laying Of Curbs, Edges And Sewage, Construction Of A Maneuvering Road Structure With Exits, Parking Spaces And Hardening Of The Area, Field Stairs, Sidewalk And Roadside, Vertical Markings And Horizontal, Lighting, Cutting Down And Planting Trees, Shrubs And Green Areas, Demolition And Construction Of The Power Grid, Demolition And Construction Of Telecommunications Networks. 5.4.1 The Contractor Will Be Obliged To Submit To The Ordering Party, Upon Each Request And Within The Deadline Specified In The Request, The Following Evidence Confirming The Fulfillment Of The Requirement Of Employment Under An Employment Contract By The Contractor Or Subcontractor Of Persons Performing The Indicated Activities During The Execution Of The Order: A) Declaration Of The Contractor Or Subcontractor On The Employment Under An Employment Contract Of Persons Performing The Activities Covered By The Request Of The Ordering Party. This Declaration Should Include, In Particular: The Exact Description Of The Entity Submitting The Declaration, The Date Of Submission Of The Declaration, An Indication That The Activities Covered By The Request Are Performed By Persons Employed Under An Employment Contract, Together With An Indication Of The Number Of These People, Their Names And Surnames, The Type Of Employment Contract And The Working Hours, And The Signature Of The Person Authorized To Submit A Declaration On Behalf Of The Contractor Or Subcontractor, B) Declarations Of Employed Employees Containing Information, Including Personal Data, Necessary To Verify Employm
