Tomography Devices – Delivery, Assembly And Commissioning Of A Computed Tomography Scanner Along With Adaptation Works. 1. The Subject Of The Order Must: A) Be Brand New, Year Of Production Min. Q4 2025, Not Ex-Exhibition, Not Previously Used For Presentations, Complete And After Commissioning, Ready To Work As Intended Without The Need To Make Any Additional Purchases And Investments, Safe For Patients And Medical Staff, B) Come From Current Series Production, Without Introducing Any Changes Or Modifications To The Device For The Purposes Of This Order Only, C) Be Introduced To The Market And On The Market Of The Republic Of Poland Before The Deadline For Submitting Offers In Accordance With Polish Law, D) Have A Production Date Visible On The Casing, E) Be Manufactured In Accordance With The Essential Requirements And Safety Standards Specified For Medical Devices In The Provisions Of European Union Law And In The Provisions Of Polish Law, F) Be Consistent With The Requirements Specified In The Act Of April 7, 2022 On Medical Devices, G) Meet The Requirements Of Polish Law On The Date Of Receipt By The Ordering Party, H) Have A Minimum 36-Month Warranty, In Accordance With The Provisions Of The Contract Template And Annex No. 2 To The Swz. 2. The Device Should Be Made Of Materials Resistant To Cleaning With Cleaning And Disinfecting Agents - Its Operational Parameters Cannot Change Under The Influence Of Such Agents. The Device Should Have Protection Against Improper Use. 3. A Detailed Description Of The Subject Of The Order Includes: A) Annex No. 2 To The Swz - Description Of The Subject Of The Contract/List Of Required Technical Parameters, B) Annex No. 3 To The Swz - Functional And Utility Program.
