5.1 Name Of The Order: Current Maintenance Of Municipal Roads In The Andrychów Commune In 2026. 5.2 The Subject Of The Order Includes In Particular: 5.2.1 Renovation Of Roads With A Stone Surface 5.2.2 Operation Of Equipment 5.2.3 Delivery Of Stone Materials 5.2.4 Maintenance Of Ditches, Culverts And Slopes 5.2.5 Renovation - Replacement Of Pipe Culverts Along With Accompanying Works 5.2.6 Installation Of Channels 5.2.7 Construction Of Run-On Grates 5.2.8 Removal Of Sinkholes 5.2.9 Repairs With Asphalt Mass 5.2.10 Delivery Of Construction Materials 5.2.11 Other Works And Works Necessary For The Proper Execution Of The Subject Of The Contract 5.3 The Description Of The Subject Of The Order Is Included In The Annexes To The Swz, I.E.: Annex No. 3 And 3.1 Technical Specification Execution And Acceptance Of Works, 3.2 Bill Of Quantities. The Contractor Is Obliged To Complete The Order On The Terms And Conditions Described In The Draft Contract. 5.4 The Ordering Party Requires Pursuant To Art. 95 Section 1 Of The Public Procurement Law, Employment Under An Employment Contract In Ones Own Enterprise Or By A Subcontractor Of Persons Performing Activities Specified By The Ordering Party In The Scope Of Contract Execution, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code, I.E.: Physical Work Including Renovation Of Roads With A Stone Surface, Equipment Operators, Operators Delivering Stone Materials, Maintenance Of Ditches, Culverts And Slopes, Renovation - Replacement Of Pipe Culverts With Accompanying Works, Installation Of Trays, Construction Of Run-On Grates, Removal Of Sinkholes, Repairs With Asphalt Mass, Operators Delivering Materials Construction. 5.4.1 The Contractor Is Obliged To Submit To The Ordering Party, Upon Each Request And Within The Deadline Specified In The Request, The Following Evidence Confirming The Fulfillment Of The Employment Requirement Under The Employment Contract By The Contractor Or Subcontractor Of Persons Performing The Activities Indicated In Point During The Execution Of The Order. 5.4: 5.4.1.1 Declaration Of The Contractor Or Subcontractor On The Employment Under An Employment Contract Of Persons Performing The Activities Referred To In The Ordering Partys Request.
