Contractors Declaration - List Of Construction Works Performed No Earlier Than 5 Years From The Deadline For Submitting Offers, And If The Period Of Running The Business Is Shorter - During This Period, Along With Their Type, Value, Date And Place Of Performance And The Entities For Which These Works Were Performed, And Attaching Evidence Determining Whether These Construction Works Were Performed Properly, The Evidence Referred To Being References Or Other Documents Prepared By The Entity For Which The Construction Works Were Performed, And If The Contractor For Reasons Beyond Its Control From Him It Is Not In
